1-2-3 FAMILY Day Care is licensed by the State of Wisconsin, Department of
Health and Family Services.  I am licensed to care for no more than eight
children at any one time.  I am inspected regularly to insure that I meet
licensing standards.  Smoking is not permitted on the premises of the center
during licensed child care hours.  My Husband Douglas does smoke on the
premises when the center is closed.

I will post the following items for your review: license certificate, results of the
last monitoring visit, any exceptions, a record of meals served.

Parents will receive a pamphlet, which is a summary of family child care
licensing regulations, "Your Guide to Licensed Child Care," as part of an
enrollment packet.  1-2-3 Family Day Care is covered by liability insurance for
my premises and/or for my operations.

1-2-3 Family Day Care does have pets on premise.  3 cats and lots of fish
they will be in the child care area.  .  All pets for which there is an effective
vaccine against rabies have been vaccinated.  The children will be closely
supervised when the animals are accessible to children to ensure that both
the children and the animals are protected from harm.  1-2-3 Family Day Care
as liability insurance that includes coverage for dogs/cats.  If your child has
allergies, please inform me.

Parents are welcome to visit my child care program at any time during the
hours of operation unless prohibited by a court order.  If so, I will need a copy
of the order.

Children will only be released to persons listed on the enrollment form.  If
anyone other than the child's parent or someone who is listed on the
enrollment form is to pick up a child(ren), I need to be notified in writing or by
a telephone call in advance.  The person picking the child(ren) up may need
to show a driver's license or other picture ID.

If the parent or other authorized person arrives to pick up a child and that
person appears to be intoxicated or under the influence of drugs all
reasonable steps will be taken to prevent the person from leaving with the
child, including offering to call a cab or another contact person.  While I
cannot legally withhold a child from the legal guardian I will not hesitate to call
the local authorities if I feel the child is in danger.

It is important that we communicate daily concerning the needs and interests
of your child.  If there are issues or concerns that need to be discussed
please work with me to arrange a convenient time to talk on the phone at
naptime or in the evening so we can give the issue the attention it deserves.  
To foster communication on a regular basis1-2-3 Family Day Care   provides
scheduled conferences/written newsletters/parent bulletin board/daily sheets.

To protect each family's confidentiality, 1-2-3 Family Day Care   will not share
information about a child or a child's family with anyone who is not authorized
to receive this information.

Parents must meet with me to discuss their child's specific needs and to
review program policies.  The following items must be completed and returned
to the center by the first day of attendance.

I will inform you of any updates that are needed and give you 30 days to
submit the updated forms.

after your child starts attending.

record of your child's immunizations
•        Food program enrolment form

Children may be enrolled on a full-time basis (40hours per week or more), or
a part-time basis (less than 5 hours per day ).  No child may be regularly
enrolled for less than 10 hours or 2 days per week.  I do accept children for
drop-in care if prior enrollment arrangements have been made and space is
available.

Thank you for choosing my services for your child.  I started this business in
1996 in order to provide quality, reliable daycare for my own child.  My goals
are to provide a positive, safe, nurturing environment to all of the children in
my care.  I realize that most parents in today's society do not have the
opportunity to stay home with their children, so I intend to offer the next best
option.
      

My Philosophy:
As a working parent, you face the difficult challenge of balancing the
demands of work with the needs of your family.  Finding high quality childcare
that is flexible to your work schedule is one of the most difficult tasks you will
face.
I strive to provide safe, comfortable, stimulating, fun, educational environment
in my home with a semi-structural day.  Activities will include free playtime,
structural playtime, large motor playtime, small motor skilled activities, small
time and theater time, arts and crafts, music and dance activities, nature and
ecology awareness, nutrition, cultural activities, and other educational yet fun
activities.  I will also provide nutritious meals, snacks, and encourage good
personal hygiene and manners.

Television will be aloud during daycare hours, but only educational shows will
be aloud. Some examples are Barney, Blues Clues, Arthur, Franklin, Little
Bear, etc...  I also have a computer that will be available to all ages but only
under adult supervision.  School age children may have internet access, upon
parental request, but only again while supervised.
Through a mixture of guided and indirect creative play, children in my care will
have the opportunity learn about color, shapes, textures, numbers, letters,
animals, seasons, feelings, senses, nutrition, personal hygiene, manners,
basic science and math concept, and creative concept such as "pretend"
play, art, music, drama and dance.  These may all sound lofty goals for
infants, toddlers, and preschoolers, but most children learn all of these things
and more through creative educational play.  I do not believe in pushing any
child to learn at an accelerated rate, and will not make a child feel pressured
to achieve developmental milestones.  By providing the right kind of fun safe
environment, I believe the children will learn at their own appropriate pace.
 
             

House Rules
1. I am not responsible for lost or broken items that children choose to bring
from home.
2. Please limit toys and books from home. Please NO small things that the
little ones could put in their mouths (including coins).
3. All children have a rest time in the afternoon.
4. There is no physical punishment in my childcare. A child may be removed
from the play group for a cooling off period of 3-5 minutes.
5. I can not accept sick children into my care. If your child becomes ill while in
my care, you will be notified and your child will need to be picked up as soon
as possible.
6. Due to the risk of injury while jumping on a trampoline, the Wisconsin
Department of Human Services, Division of Child Care Regulations prohibit
the use of a trampoline by children in a home daycare facility.  While your
child is under my care he/she will not be permitted to use this equipment.

7. If I must be away from the daycare, i.e. Doctor’s appointment, for a short
period, my husband Doug, will provide substitute care.




TRAININGS/AFFILIATIONS

I have been a family childcare provider since March of 1996.  I have
completed my Licensing Orientation to become a licensed daycare home for
the State of Wisconsin and participated in the state's Mentor Program.  The
Collage Courses that I have completed are Child Nutrition Mod 1-2, Infant and
Toddler Basic Care, Child Care Teaching Methods, Mentors and Protégés at
work, Family daycare Licensing, Marriage and family, Nutrition Health and
Safety, Family Daycare Management 1-2, I have also completed orientation
for certification in Grant County WI.  I have attended educational workshops
pertaining to child development and have received a mini-grant from CCR&R
to put toward the improvement of my daycare.  I have also attend WFCCA  
(Wisconsin Family Child Care Association ) conferences to continue my
education.  Of course I am CPR and First Aid certified. Other Trainings
include  Mentor Protégé’ Teacher Program, 3 hours yearly nutrition classes,
Child Care Business Partnership program, The Nurturing Program,
              

PURPOSE AND GOALS

My goal is to provide a safe and secure environment where love abounds and
where children can explore and learn from the world around them.
                 

PHILOSOPHY OF LEARNING

My philosophy is that a warm and nurturing environment is made rich by
“hands on” play-learning opportunities.  Children learn about themselves and
their environment through their surroundings.

From birth, babies are marvelous learners, immediately investigating the
sights, sounds and feel of the world.  Long before walking and talking, they
are exploring their own bodily powers and what the world has to offer.  
Though much of a very young child’s play, learning is self-motivated and self-
directed, it is still necessary for caregivers to stimulate the child through
individualized play-learning experiences in an enriched environment.

Children of all ages are given opportunities to learn through experiences with
stories, songs, art, science, dramatic play, sensory materials and math
activities.  A preschool program designed for Family Childcare will also be
provided for children ages 2+.  Younger children will be considered at the
parent’s request.
                     
           

OUTDOOR PLAY

I believe that in order for children to be healthy, they need fresh air and
exercise.  As a caregiver, I am required to provide outdoor time for children
everyday, weather permitting.  Parents are advised to be aware of daily
weather conditions so that appropriate outdoor wear is sent to my daycare
with the child.  If your child has frequent ear infections, please be sure to
send adequate covering for his/her ears.  Children will be taken outdoors in
hot and cold weather and I will monitor the conditions and limit the time
children are outside to assure that children are neither too hot nor too cold.
            
COMMUNICATION WITH PARENTS

Keeping close communication with my families is a priority!  Because we may
not always have the time to talk about how and what your child is doing, I
have established the following communication system to help us keep in touch
at all times:

~Daily Reports~
Each child will have a daily written report detailing their activities and
routines.  Infants will have a journal (if two-way communication is maintained
via the journal) detailing their day; again, two-way communication between
caregiver and parents is strongly suggested by way of the journal.

~Entryway Posting~
I will post information regarding upcoming events, reminds, on the wipe off
board below the time clock!

~Parent’s Board~
In the kitchen, there is a Parent’s Bulletin Board which contains our monthly
calendar, curriculum information, community information and other fun tidbits.

~Mail boxes~
Each child will have a file accessible to parents.  This is where receipts and
notes to parents will be placed.  

~Parent Conference~
occasionally parents will be invited to meet and discuss their child’s
development and concerns.  This may be initiated by either the parent or
myself.

~Calendars and Newsletters~
A monthly calendar and a monthly newsletter will be distributed to keep you
informed about learning themes, special dates and topics of interest.  The

~Evaluation Forms~
I encourage you to give me feedback about my program through
conferences, daily conversations or surveys in a formal evaluation.  Written
surveys are given to each parent twice a year.  Please be honest as I will use
this information to better my program.

PARENT PARTICIPATION

Occasionally I will invite parents to some special events each year such as
their child’s birthday party or special holiday parties.  Additionally, you may
participate by helping with a field trip or in any way you like.  Of course you
are always welcome to visit at any time during the daycare day!

All immunizations must be up to date. Any child found not up to date will have
24 hours notice to receive required immunizations. The provider can refuse to
care for any child(ren) if required immunizations have not been received.

I will plan activities and provide children with a variety of experiences.  Some
of the activities will include:

Language development: Books, music, story time, fingerplays, flannel board
stories

Large muscle skills: Balls, hula hoops, bean bags, swinging, outdoor play

Small muscle skills: Art and crafts activities, stringing beads, Legos,
pegboards, blocks

Creative expression: Dramatic play, puppets, music and musical instruments,
art activities

Self-help skills: Assist with mealtime preparation, dress self for outdoor, help
with jobs in the center

Play is the major component of our program.  Enough time, materials and
space will be provided for children to actively explore the world around them.  
Children will have an opportunity to use a variety of art materials,
manipulative and housekeeping equipment.  Children, including infants and
toddlers, will go outdoors daily when weather permits.  The children will be
kept indoors if the the temperature is below 0 degrees including wind chill;
raining or storming.

I will allow children to watch television including Videos or DVDs.  Children
may not bring videos or DVDs from home.  The children will be allowed to
watch television before naptime, during the early morning and late afternoon
or on special occassions.  Children are not required to watch television and
other activities will be available during that time for children to use.

School age children will have a quiet place to study or relax, access to
appropriate materials and activities, and will have ample time for large muscle
activities and to participate in food preparation.

Children under two years of age will have a flexible schedule, which reflects
the child's individual needs.  They will be given individual attention including
lots of time for talking.  The body position of non-mobile infants and their
location in the center will be changed frequently.  I will provide safe, open
spaces for children whom are creeping and crawling.  Infants and toddlers will
be encouraged to play with a wide variety of safe toys and objects.

             
           Pets:
We love animals here, and so we have daycare pets! We have fish that the
children enjoy watching & feeding, and we also have 3 cats they have been
raised in our daycare and they feel that they are just like one of the kids.
They are very gentle and love the children. They are up to date on all their
shots. You will be notified at least 2 weeks in advanced if we get another pet!
Excluding fish!

              
TOYS
Please DO NOT allow your child to bring toys from home, as it tends to cause
conflict with the other children. The exception would be item for quiet time
such as a favorite stuffed animal or blanket. This will only be allowed at
naptime.

Number of Children in Care

I believe that children receive the best possible care when there are eight (8)
or fewer children under the age of 12 per adult.  Except with prior agreement
from all parents using my services, I will not care for more then one infant
(under 12 months) at a time.  When it comes to filling vacancies or increasing
the number of children I care for (a maximum of 8) I will always give
preferential consideration to the families I currently serve.   I would currently
welcome the opportunity to care for new siblings as they arrive.

Children need to feel that they are getting quality attention from the adults in
their lives, and they also benefit from lots of one-on-one time.  Therefore, for
the children's' benefit and mine, I will attempt to keep the group as small as
possible.  From time to time in order to accommodate the families that I serve,
it may be necessary to accept one or two extra children for a day or two, such
as in the case of a school closing due to weather conditions.  On these days I
reserve the right to accept older children for temporary care decide additional
fee.  To the extent possible, I will notify all parents in advance of such days,
so that you can decide if you wish to keep you child in care that day.  Be
assured that I will never accept more children for care then I believe would be
safe and in the best interest of all the families that I serve.  I anticipate that
this will be a rare event and not a routine situation.

Occasionally I will ask my mother to help watch the children in my home or
during a prearranged field-trip.  You will have the opportunity to meet her,
and I will always attempt to let you know in advance when she will be assisting
me.  I will also welcome your assistance during special field trips or other
special fun events.  I realize that you would not be hiring me for care if you
regularly had time to spend daytime hours at home, so this time would be
purely voluntary.  Of course you are always welcome to drop by anytime
during regular business hours.
                 
Arrivals & Departures  

Children are to arrive clean, dressed and fed (unless arriving just before a
meal time).  It is normal for some children to have difficulty separating from
parents, or cry when being dropped off.  Please be very brief (no more than 5
minutes) during drop-off times; the longer you prolong the departure, the
harder it gets.  A smile, cheerful good-bye kiss, and a reassuring word that
you will be back are all that is needed.  In my experience, most children are
nearly always quick to get involved in play or activities as soon as parents are
gone.

Please be brief at pick-up times also.  This is a time of testing when two
authority figures are present (the parent & provider), and all the children will
test to see if the rules still apply.  Typically pick-up times need to be kept
brief.  During arrivals & departures, I expect you to back up my House Rules
(posted below), but if you do not.  I will remind your child that their behavior is
inappropriate and take action to correct, if necessary.  Please be in control of
your child during these times.  Do not allow your child to run out to the car
while you are still inside!  The safety Rule is “No one goes outside without an
adult with them.”

Sometimes, at pick-up time, the children may get a little over-excited and/or
rowdy because someone else (parents) are here. Please help show your child
that you respect me and my rules by reminding them (and allowing me to
remind them, if needed) that the rules still apply whether or not someone else
is here.  Or whether or not you are here some children act out to get their
parents attention and that tends to get the whole group started. (There is a
$1.00 per minute early drop off, and late pick up charge. )  It is due on the
next schedules day of childcare.  

Tips for Smooth Transitions at Drop-Off and Pick-Up

Transition times can be stressful for some children. In the morning they may
not want to say good-bye to you and at pick up time they may not want to go
because they are involved with a fun activity. This is a natural occurrence and
may vary with your child’s age and developmental stage.
To ease the situation we encourage parents to take a minute to help their
child feel comfortable and to adjust by offering positive statements. A parent
who shows reluctance to depart only makes the transition harder for their
child. Although parents should never sneak out without saying "good-bye" to
their child, a brief good-bye usually works best. Crying will usually stop a few
minutes after you depart.  Children of all ages adjust to transitions differently.
When you arrive to pick up your child, we will let you know briefly how their
day went, any special events or accomplishments, eating, sleeping and
bathroom habits because the children, as well as the provider and/or
assistants are all anxious to end our long day care day.  If you have any
concerns and/or issues, I ask that you call and arrange a time to discuss the
matter.  Some concerns and issues should not be discussed in the presence
of the child, children and/or other parents.


Open Door Policy

I maintain an Open Door Policy.  This means you are always welcome to call
or drop in to see your child at any time during regular hours. You are required
to let me know of your presence before entering my home. Knock, and then
enter.  You can enter after you knock and do not have to wait for me to
answer the door.  Open Door Policy does NOT mean my door will always be
unlocked.  For children’s safety, I would not want unwanted or unexpected
visitors to enter without my knowledge.  I also do not want little ones leaving
the house unsupervised.  Your child’s safety is my first priority.  Please
remember when visiting children may react in an excited manner that does not
normally occur when I am alone with the children.
                 
Absences

There will be no refunds or adjustments made to your childcare fees for your
time missed due to illness, holidays, vacations (yours), or days off.  A place
has been reserved for your child that cannot be filled on a short-term basis.  
Although I will make every attempt to be available each day, there will be
occasions when I or another family member is ill and I am unable to provide
care. You will be notified as soon as possible of any personal or family illness,
funeral, or emergency. It will be your responsibility to obtain alternate care on
those days.  Please notify me no later than 9 a.m. if you will not be bringing
your child or will be arriving late.  More than 3 consecutive absences without a
call may be considered a termination by Parent with all fees due & payable.


               
Maternity/Extended Leave:

In cases of your potential absence due to maternity leave where you will be
placing your new baby into my care, all forms must be filled out prior to
starting care.  Also, half the regular weekly fees will be due for the new baby’s
slot to be held.  Also due will be regular fees for any other children enrolled
under my care whether they attend during this time or not.
                  
Personal Belongings & Parent Provided Supplies:

No toys should be brought from home.  Little ones have a difficult time sharing
with others, and it is even harder with their own special toys.  Exceptions will
be their “cuddly” for naptime.  Parents are required to supply diapers/training
pants, wipes, formula (if you choose not to use mine) or breast milk and a
change of clothes.  In addition if your child requires a special diet, you must
supply it.

I will send home a notice informing you when your child's supplies at daycare
are running low.  I will also keep extra diapers and wipes here at my family
childcare so that no child would ever have to do without.  If you do not send in
supplies to restock your child's shelf within two days of your written notice, I
will charge a fee of $1.00 for each diaper that I use from my personal supply
for your child.  Fees will be added to the weekly tuition amount and need to
be paid in full by the end of the week

Property brought to day care is not my responsibility.  I cannot and will not be
responsible for lost or broken items.  If the children are unable to share, their
item(s) will be put away for the day.  If a favorite cup, cuddly or a similar items
is brought with your child you will need to make sure you take it home if you
wish for it to go home otherwise cups are put in the kitchen and favorite items
are in their cubbies.

Any property that remains 10 business days after the last day of the child's
attendance becomes the property of the Lil Angels Home Day Care to be
disposed of as I see fit.  Please take your child's belongings on their last day.

                        


Releasing Your Child
My normal procedure is to release the child only to his or her parents or
someone else the parents designate. If someone other than the parent is to
pick the child up, please notify me ahead of time. A verbal notice is fine on
that day if this person is on the list of those authorized to pick up your child. If
the person is not on that list, I must have written permission to release your
child.
One of the forms you are required to complete designates who may pick the
child up if there is an emergency and you can not contact me. Please make
sure those listed are persons with whom you would allow your child to leave if
that person showed up at my door and said, "I need to take Johnny with me."
Those on the list should also be people I could call in the event something
happened and you did not show up to pick up your child.
Please inform your emergency contacts that if I do not know them and the
child is too young to recognize them ("Hi, Grandma!"), that I will ask for
identification. I do not mean to offend them. This is simply a measure taken
for the child's protection.
               
Infants in Care

Since 1992 the American academy of pediatrics has recommended that
infants sleep on their backs to reduce their risk of sudden infant death
syndrome (SIDS). But some families still place infants on their stomachs to
sleep. In order to provide the best care for your infant the daycare does the
following:
•        Infants under 12 months will be placed on their backs on a firm, tight
fitting mattress for sleep (in a portable  crib)
•        Waterbeds, sofas, soft mattress, pillows, and other soft surface will be
prohibited as an infant sleep surface.
•        All pillows, quilts, comforters, sheep skin, stuffed toys, and other soft
products will be moved from the crib.
•        When a blanket is used, the infant will be placed at the foot of the crib
with a thin blanket tucked around the mattress, reaching the infants chest.
•        The infants head will remain uncovered during sleep.
•        Infants will be places in a supine (back) position for sleeping to lower
the risk of sudden infant death syndrome (SIDS). (Unless the child has a note
signed from a physician specifying otherwise)
•        When infants can easily turn over from the supine to the prone position,
they will still be placed on their backs but allowed to adopt whatever position
that they prefer to sleep.  Unless a doctor specifies the need for a positing
devise that restricts to movement within the child’s crib, such devices will not
be used.

By signing this agreement, it means that you have read and understand my
rules and reasoning’s to how I will be laying all babies down to sleep in my
daycare .

Nap/Quiet Time:

All children are required by State guidelines to have a rest period each day.  I
do not require older children to sleep; they must however lie quietly until the
other children are asleep and then may quietly play or look at books.  School
age children may play outside or quietly in the playroom until rest time is
over.  I do sometimes offer movies during this time for children as well.

1-2-3 Family Day Care   will provide care for children between the ages of 2
weeks and 12 years years.  I will never refuse to enroll a child on the basis of
race, sex, color, creed, political persuasion, national origin, handicap,
ancestry or sexual orientation.  All children will be enrolled for a trial period of
2 weeks.  During the trial period either the provider or parent may terminate
child care without advance notice.

A child may be discharged from the center for reasons such as, but not
limited to:

        Failure to pay fees on time.  (Grounds for immediate termination,
without advance notice.)

        Lack of parental cooperation.

        Inability of child care program to meet the needs of the child.  I will
consult with the parent concerning how any problems might be solved before
ending the care arrangement.  The parent will be referred to other community
resources.

        Repeated failure to pick up the child at scheduled time.

        Failure to complete and return required forms.
Parents must give a 2 weeks written notice of their intent to withdraw the child
(ren), and will be required to pay for those 2 weeks, whether or not children
continue to attend.  All outstanding fees must be paid.

I will give a 2 weeks written notice of my intent to discharge a child (and try to
inform parents of local resources that may be of help to them), except when
due to parent's failure to keep current with fees owed.  Should parent remove
the child during a notice period I initiate, fees will not be charged for
remaining unused days.
                
Tuition and Hours of Care

Current fees appear on the rate sheet.

My services will be available Monday through Friday from 5:00 a.m. till 5:00p.
m. Please discuss needs for care outside these times and I will attempt to
accommodate reasonable request at a possible additional fee.  (Although I will
do my very best to be available every business day, it is conceivable that I
may be forced to close occasionally due to vacation, illness, or other
emergency.  If you cannot ever tolerate care, but possible lapses in care, you
should consider placing your child in a daycare center, or make other
arraignments for backup coverage.  I can recommend several options if you
need referrals.

Unless other arraignments are made in advance, charges for the next week’s
care are due on Monday for that week of care.  
. (There is a $1.00 per minute early drop off, and late pick up charge. )  It is
due on the next schedules day of childcare.  

Annual Enrolment Fee
I will be charging a 25.00 per child enrollment fee (must be paid on enrolment)
This is to help cover the cost of summer pool passes, office supplies, travel
expenses, ect…

Tuition rates are according to the following hour categories:

State Funding:  I do accept state funding for childcare.  Please contact your
worker as soon as possible to give them my information.  I need to have a
Notice of Decision so I know what hours the state will accept for billing.  Any
hours over and beyond what the state pays for is your responsibility and you
will be informed of any hours you went over.   All co pays that the state says
you need to pay are due on Mondays.

Fees are to be paid in advance on Monday for the following week's services.

If there will be a third party payment, as from an employer or the county, a
special payment schedule will be arranged and detailed in the contract.  
Parents will be responsible for any specified co-payments or unpaid amounts.

There will be no reductions for additional children from one family.  I am
limited to the number of children that I may care for at one time.

40 hours a week is    $100.00
45 hours a week is    $112.50
50 hours a week is    $125.00
55 hours a week is    $137.50
A late fee of $10.00 per day will be added to weekly tuition charge, unless
prior arrangements are made.  Late pick up charge . (There is a $1.00 per
minute late pick up charge. )  It is due on the next schedules day of childcare.  


I will attempt to be generous on this sense I have a working husband and I
realize job pressure, traffic, and weather conditions can occasionally affect
your ability to arrive on time,  however, sense a habit of arriving late does
affect my family life, I will need to change late fees if I feel a pattern of late
arrival develops.  Please feel free to discuss arrival and drop off times with me
if your needs vary, such as when you or your spouse needs to travel for
business.  I will attempt to be flexible to meet your needs for when you or your
spouse needs to travel for business.  I will attempt to be flexible to meet your
needs for occasional early arrival and late pick-up times, but these will need
to be exceptions rather than a general rule so as not does unduly impact my
own family.

Payment is preferred in cash; however, checks and money orders will be
accepted when drawn from a local bank.  In the unlikely event a returned
check, I will require a $25.  Service charge in addition to the of any bank
charges I may incur as a result of the inconvenience.

In order to provide my family with a stable financial situation, I cannot offer
any discounts for days your child is not in my care, such as when you are on
vacation, or when your child is ill.

(After being in my daycare for 6 months there is eight (8) days that you do not
have to pay for.  You can not use them as a 2 week notice.) Although I realize
daycare is a major expense for most working parents, I also rely on this
business to help support my own family.  Since I will be holding a space for
your child for you to use at your will, I need to charge for full time care
weather you use it or not.  Rare exceptions may be made at my desecration
for extreme circumstances.

You will always receive a receipt upon payment for my services.  At the end of
the year you will receive a proper tax form that shows the full amount of all
payments and charges for that calendar year.

Child care services will be provided between the hours of 6 A.M. and 6 P.M.,
Monday through
Friday.




  



Field Trips:

Field trips (2-3 times a month) we twill be traveling to local libraries, post
office, fire station, police dept, bakery, newspaper shop, other factories,
hospitals, post office ect…  I feel no matter the Childs age they learn from
hands on.  I feel children can and will learn so much more with the experience
right in front of them and letting them reach the learning opportunity to learn
Parents are always welcome to accompany us on trips.  And sometimes I will
ask for a helper or two for these types of trips in my newsletters so feel free to
volunteer or have a friend or relative help out if you would like.

Signing this contract implies that you are allowing me to transport your child in
the proper restraints in my vehicle for field trips, various outings, and school
drop off and pick ups.
                       
Photos/Newsletter:

I do have a digital camera and at times will take photos of the children doing
fun activities and may use them in my newsletter, Bulletin Board on Living
room wall and on my web page for advertising my daycare.  You may view my
webpage at anytime I will provide you with the web address.  If at anytime you
would like to see photos that have been taken of you children feel free to ask
and I will provide them to you.  By signing this you give me permission to use
the pictures for advertising and daycare use.  
               
clock In/Out:  

All children must be clocked  in and out.  There is a Time clock and time cards
in a file in the entryway on the cubbies. Each child has their own time cards.  
Please clock them in and out with the current time and your initials.  If they are
picked up or dropped of by a bus I will sign them in or out at that time.  Use
the "back of the card for notes to me!
" column to make note of important information (I.E.: a morning dose of
medication, early pick up time, dr. appointment, new work phone number,
etc.).  This is done for tax and legal purposes so that I can show that your
child was here during a certain time frame.
              
Custodial Agreements:

Clients that have Custodial agreements would be wise to provide a copy to
me for safety reasons.  All arrangements are different and for the protection
of your child, your family, and my daycare this will assist if a parent shows up
to take a child that has no right to have that child.   I can’t release a child to
another parent without permission from the client that I hold the contract with
unless I have a signed consent form.
                
Discipline:  

Discipline is used to teach a child, not as a punishment.  I achieve this
through love, consistency, and firmness.  I stress two main patterns of
behavior: respect for themselves and other people and respect for property.  
The children are explained the rules of the childcare home frequently, so they
are all familiar with the guidelines.  The following methods of discipline will be
used:
        Encourage children to solve problems themselves, Intervention and
discussion, Re-direction to another play area, Loss of privileges, Time out.  If I
feel there is a chronic behavioral issue that needs attention, I will let you know
so that you and I are handling it in the same way so there is continuity in
discipline between our homes.  These types of behaviors might include such
things as biting, use of bad words, chronic hitting, name calling, defiance,
breaking toys, etc.  Together, we will try to find a solution.  You may be called
to remove your child if his/her behavior prevents me from being able to
properly care for the other children.  If the problem continues, other
arrangements for the care of the child will have to be made, for the safety and
well being of all.  Repeated unresolved discipline problems will be grounds for
immediate termination.
Please respect my profession, my home, my family and me.  The respect you
show me, including my home, furnishings, equipment, yard and other children
will communicate itself to your child and make for a better working
environment.  There are certain House rules that all children will be taught
and expected to follow.  In addition, although I realize that I must expect a
certain amount of wear and tear where children are concerned; I do not want
my home “demolished”.  The House rules are as follows and are enforced for
the safety and well being of everyone.
                
I expect all children (with age taken into consideration) to know the rules of
the house.

1.  No hitting, kicking, biting, pushing, etc.
2.  No throwing or intentionally breaking anything.
3.  No running, jumping, wrestling, climbing, etc. in the house or on the
furniture.
4.  No shoes on the couch. Remember that furniture is for sitting.
5.  No picking up babies or toddlers (without permission from an adult).
6.  No leaving the yard or house without permission (all children under 5 are
not allowed outside without supervision - even when mom/dad is here).
7.  No name calling, teasing, yelling, etc., everyone deserves to be treated
with respect.
8.  Please remove shoes at the door. (Older children should leave an older
pair of tennis shoes here to play outside in during the winter - this will save
their better shoes, my carpets, and you a lot of laundry!).
9.  No children are allowed in the garage or basement area.  Children must
also ask permission before entering Christopher’s and/or Mariah’s bedroom
or my bedroom.
Please do not bring any food to day care unless you bring enough for
everyone! PLEASE! Do not let your child walk into the house carrying ANY
food, cups, or bottles! They always end up on the floor, in the playroom, etc.
Please, for the sake of the carpets, clothes, and hair, do not bring gum!

Rules for parents:

1. No cars allowed in my driveway if leaking oil. Please park along the street
2.  Be polite to all children in house.
3.  No food, drink, or gum brought into the house for your child. It will be
disposed of.
4.  Always come to the front door unless there is a note posted there stating
otherwise.
5.  Knock prior to entering.
6.  No shoes in the house.
7.  No smoking is permitted in the house.
              
Holiday, Vacation and Sick Days

I realize the need for reliable daycare and will attempt to give as much notice
as possible whenever I must close my home to care for any reason.

In order to spend quality time with my family, I will be closed for the following
major holidays:

I will also take 10 days off through out the year vacation or sick days with pay.
(Then I will not require payment for my unscheduled days off)  Working with
exposure to many families and especially children, it is expected that I will
become ill occasionally.  Therefore, I will always attempt to give as much
notice as I can when I am forced to close down my daycare due to illness.  
Also, in event that one of my family members is ill, I will also give you as much
notice as possible to allow you to decide if you want to risk the exposure to
your child.

                    

If Your Child Becomes Ill

If your child will not attend on a regularly scheduled day please let me know
by 8  A.M.

I will transport children who attend school at Boscobel Elementary to and from
the center in the morning and after school.
If parents wish to allow a school-age child to leave or arrive at the center
unescorted, they must provide written authorization for this activity.  School-
age children who leave the center unescorted must be traveling to home,
school or another activity where adult supervision is present.  

Although I am not a trained professional, I have been educated in recognition
of infectious childhood diseases and will attempt to alert you at the first sign of
any illness.
In order to protect the health of all children in my care, I will call you at work
and request that you pick up your child within one hour if he or she exhibits
and of the following symptoms:

•        Fever over 100 degrees for more then 1 hour
•        Excessive diarrhea for 2 or more consecutive diapers or toileting
•        Vomiting in excess of typical infant spit up
•        Conjunctivitis (pink eye)
•        Consistent complaints or ear or stomach pains
•        Bleeding other then minor cuts and scrapes
•        Excessive greenish nasal discharge, indicating possible infection
•        Head lice
In event that your child is sent home due to one of the above conditions he or
she is not aloud to return until accompanied by a signed by the dr.  This is
necessary to prevent the unnecessary infection of the other children in my
care.  Although it may seem inconvenient when your child is sent home, you
will appreciate knowing your child's exposure is minimized when the other
children become ill.

I will not send a child home with a common cold.  This sort of illness is going to
accrue among children, and unfortunately cannot be totally avoided.  You
must expect that anytime a new child is introduced to the groups, cold are
likely to accrue.  I will do my best to prevent the spread of such illness through
hygiene and house keeping, but totally avoidance of this sort of thing is
impossible.  

Likewise, I will not close my daycare if I have a mild cold. I will, however notify
you as soon as symptoms appear in myself or any other children, so that you
can decide whether or not to have your child in care that day.

I will follow procedures on personal cleanliness and communicable diseases
with licensing rules and the guidelines for exclusion of children from child care
as adapted from the Division of Public Health.

                     
Administer Medications

Since I am not a licensed medical professional, I will not administer any type of
medication including prescription and over-the-counter medications, without a
signed consent form (see attached).  This form can be filled out when you
drop off your child and must include:

•        The child's name, age, and weight
•        The name and the phone # of your child's doctor
•        The name of the medication
•        The dosage amount and frequency
•        Instructions for proper storage and preparation of the medication such
as should it be refrigerated, shaken, mixed with water etc...)
•        Reasons for needing the medication (so parents can be alerted to
contagious illness)

Also, the medication must be clearly-labeled in child proof container.  I would
prefer you also bring the medication in a small sack or baggy labeled with the
child's name. This is to avoid any possible confusion.

Although I am trained in infant and toddler CPR, basic first aid, and
recognition of communicable childhood diseases, I do not pretend to be a
doctor, and will not under any circumstances provide any medications,
including vitamins.  You must provide any and all medications for your child.
                    
Discipline Policy

I do not believe in spanking, slapping, yelling, or hitting of any kind. This type
of discipline will not be used in my home, regardless of your practices in your
home. Instead, I use the following discipline methods to handle any
dangerous or harmful offense:

For children under the age of 18 months:

I find it most effective to remove the child from the situation, and redirect their
attention elsewhere. Although I will most likely explain to the child that the
offending behavior was inappropriate, children of this age are rarely able to
fully understand what they have done.  Fortunately, their attention span is
also shorter than the older children, so simply showing them a different toy or
activity usually does the job.

For children of about age 2 and over:

I will alert the child to the offense by saying firmly "no", explaining why the
behavior is unacceptable, and offering the child the choice to behave.  If the
child chooses not to behave appropriately, I will first explain to the child why
their behavior is not acceptable, and then I will place the child in a chair for
"time out” about 1 minute per year of age. At the end of the time out, I will ask
the child if he or she understands why they had a time-out , ask if they
understand why they shouldn't do that behavior, then ask if they are ready to
play nicely again.  This method not only stops the offending behavior, but
also teaches the child consequences, responsibility, and empathy in a
positive manner. Dangerous and hurtful offenses including hitting, biting,
kicking, pushing, hair pulling, throwing objects at someone, climbing on an
inappropriate structure, willful destruction of property, tantrum throwing, and
not following reasonable requests.  These relatively minor offenses will be
handles as described above, depending upon the child's age and
developmental stage.

To a point, kids will be kids- but only as long as the emotional and physical
safety of all the children in my care are protected.  In rare instances that I feel
any child in my care has serious discipline problem; I will request a
conference with the parent(s).  If an understanding cannot be reached, I
reserve the right to terminate our contract with as much notice as possible in
order to guarantee the comfort and the safety of the other children in my care.

            

GUIDANCE / DISCIPLINE

Children's behavior will be guided by setting clear limits or rules for children.  I
will talk with children about expected behaviors and model those behaviors
consistently for them.  I will state positively what children can do, using
specific terms; e.g. "you need to walk" rather than "don't run").  Undesirable
behavior will be redirected to another activity.  Children will be given a wide
variety of age-appropriate activities to choose from and will be given the
attention they need before they demand it.  Behavior management will be for
the purpose of helping children develop self-control, self-esteem and respect
for the rights of others.

I do use "time-outs" to deal with unacceptable behavior.  A "time-out" or "take
a break" may be used when other techniques have not been successful.  A
time-out will be used to remove a child from a situation that has gotten out of
control before a child could hurt himself or others.  Time-outs will never
exceed five minutes and may not be used with children under three years of
age.  When used, the time-out will immediately follow the behavior.  I will stay
with the child and talk about what behavior was unacceptable, and what else
he/she might have done or said instead.  Rather than use a specific time-out
chair or corner, I will have the child "take a break" near the others, so the
emphasis is on relax/cool down, rather than isolation and punishment.  The
child will be praised after completing the time-out, and will be helped to rejoin
the group.

I recognize that no single technique will work with children every time.  If a
child exhibits unacceptable behavior, I will request a conference with parents
to consider how to deal with the behavior.  If the behavior continues, the next
steps may include referrals to appropriate community resources, and/or
discharge of the child from care.

In accordance with "Wisconsin Rules for Licensing Family Day Care" actions
that are aversive, cruel, humiliating, and actions that may be psychologically,
emotionally or physically painful, discomforting, dangerous or potentially
injurious are prohibited.  Prohibited actions include spanking, hitting,
pinching, shaking, slapping, twisting, or inflicting any other form of corporal
punishment on the child; verbal abuse threats or derogatory remarks about
the child or the child's family; physical restraint, binding or tying the child to
restrict the child's movement or enclosing the child in a confined space such
as a closet, locked room, box or similar cubicle; withholding or forcing meals,
snacks or naps; punishing a child for lapses in toilet training.  These forms of
punishment will never be used, even at a parent's request.

                    
Child abuse

Unfortunately, this is a topic that must be addressed in today's society.  It
goes without saying I will not knowingly allow any form of sexual, physical, or
emotional abuse of any child in my care at any time. This includes the child
when in their own home.  On the surface, this may sound like an invasion of
privacy, but I cannot ethically turn a blind eye toward such abuse.

I have been trained in the recognition of all forms of child abuse, and will do
everything in my power to prevent any instances from occurring.  If I notice
any signs of abuse at any time, I will alert the child's parent(s) immediately
both verbally and in writing, along with a full description of the symptoms.
(Bruising, sores, sudden extreme behavior changes, exedra.)  I will document
the same information for myself, and if necessary, alert the proper authorities
after first attempting to handle the situation discreetly.

You may get notes about your child having bruises and scrapes without
needing to fear that I suspect you of abusing your child.  Be assured, if I think
your child has been abused, I will let you know!  The notes that I send home
are just to alert you in general of any instances that may come up, and also to
protect myself from mistaken accusation of abuse.  I would prefer not to have
to deal with this issue, but I think it is best that we handle it openly and
honestly to avoid any possible misunderstandings.  We both want what is best
for our children. Please feel free to discuss this or any other issues with me at
any time.  
                 
Smoking

Licensing rules prohibit smoking anywhere on the premises (inside and out)
while children are present.  So please if you are a smoker please leave your
cigarettes in your vehicle.
              
Suspected "Under the Influence"

If I suspect you are under the influence I will contact one of the people on
your emergency contact list (if I cannot get a hold of somebody on that list) I
will contact the Boscobel police department.  If no one responds to my call I
will contact grant county dept. of social services.  (This is so I am not held
responsible for your actions)

                                    
Meals and snacks

Daycare will provide meals and snacks, with the exception of infant formula or
breast milk.  You are certainly welcome to provide any favorite foods your
child may have, but please provide enough to share with all the children. The
Daycare is on the 4 c’s food program which I am reimbursed once a month.

I will follow USDA guidelines when planning our menus.  I will offer the
following meals and snacks to all children in attendance at the times identified
in the daily schedule.  

0        Breakfast
0        A.M. snack
0        Lunch
0        P.M. snack
0        Dinner
0        Night-time snack

School-aged children will be offered an afternoon snack upon return from
school.

Monthly records of meals and snacks are available for your review.

Parents providing their own children's meals and snacks will be informed in
writing of the USDA nutritional requirements.

Children younger than 12 months must be served formula or breast milk,
unless written direction is on file from the child's health care professional.  I
will provide Similac advanced formula.

If your child has special dietary needs or has food allergies parents must
notify the center in writing.

Meal times:
Breakfast: 7:30 a.m. – 9:00 a.m.
Snack: 10:00a.m.
Lunch: 11:00 a.m.-12:00 a.m.
Snack: 3:00 p.m.
Snack: 6:00 p.m.
Sample breakfast: milk or 100% juice, whole grain toast, or muffin, and fresh
fruit.  

Sample snack: milk or 100% juice, whole grain crackers and cheese or fruit,
occasionally, I will serve cookies such as fig bars or other home made fruit
cookies, however highly sweetened snacks such as cool-aid, soft drinks,
cakes, candies will rarely if ever be offered during daycare hours.

Sample Lunch: Balanced meal such as:
Milk, a main course such as a healthy sandwich on whole wheat bread
(including peanut butter and jam sandwiches, grilled cheese, bean burritos,
etc)  or real macaroni and cheese with vegetables, whole grain pastas with
spaghetti-style sauces, nachos with real cheese, beans  and vegetables,
vegetable lasagna, and home made soups etc..

A vegetable side dish such as veggie sticks and dip, or other steamed
veggies, or a salad

A fruit serving (fresh whatever possible) such as apples, grapes, bananas,
kiwi, oranges, raisins, strawberries, melon balls, ETC.

All food will be cut up in safe-size pieces in child-size portions.  Children may
always have seconds upon request, and no child will be forced to eat
anything.  It is my experience that children will eat when they are hungry, and
will get all of the nutrients as they need when consistently present with a verity
of healthy choices.
                    
Emergency Procedure

In case of an emergency that would require an evacuation, children will be
evacuated through the nearest exit.  The attendance form and list of phone
numbers for parents and emergency contacts will be taken out to assure that
all children are accounted for and all families notified.  Children will be
assembled at 309 Pearl St.

In the event of a tornado warning, the children will be taken to location.  
Blankets, a portable radio and flashlight, with extra batteries for both, are kept
in the tornado shelter area at all times.  The attendance form and emergency
contact information will be brought along.

In the event of a lost child, I will check all areas of the center.  If the child
cannot be found, the child's parents and/or emergency contact and the police
will be immediately notified.

If a child who is scheduled to arrive at the center, via transportation other
than the parent, does not arrive within 1 hour of the specified time on the
written agreement signed by the parent, I will call the parents and let them
know the child has not arrived.

If the center should lose the use of heat, water or electricity before the center
opens; or while children are in attendance, I will call parents and ask them to
pick up the children or I will call parents by 6 AM and ask them not to bring the
child that day.

If we are unable to re-enter the building after a necessary evacuation, I will
take the children to  Casey’s Gas Station and contact parents as soon as
possible.

There will be a posted drill and evacuation plan for the following: tornado, fire
and serious injury or illness. We will have practice drills for the above
according to the State Licensing Requirements. In the case of a power outage
due to inclement weather, we will contact the parents so we can decide
together what would be the best plan of action for your child.
For life-threatening emergencies, we will call 911 for assistance and
accompany the sick child to the hospital by ambulance, leaving the other
children with a competent adult caregiver.  (Gena Brown) For non-life
threatening emergencies, parents or other listed adults will be phoned to take
the child to the doctor or emergency room.
My plan for evacuating sleeping children between those hours is to wake
children and escort them to the nearest exit.

Fire and Tornado evacuation plans will be practiced monthly.

        
   
Insurance

1-2-3 family daycare is covered by liability insurance, both on my premises
and operations.  
          

Preschool Program               
Beginning at a willing age, usually around 2, I offer a preschool program.
There is no additional cost for this program. I am currently using the program
entitled, “Carols affordable curriculum ”.  Take a moment to view this program
online at http://www.carolscurriculum.com.  We use the program 5 days per
week.  We really enjoy the program because it is flexible, fun, educational,
and it allows us time to enjoy other activities too. There is also a parent’s
letter that is sent home with you each week which includes some of the songs,
books, etc. so you can participate along with us at home! The preschool
program runs September through May. The summer months are packed full
of fun and stimulating outdoor activities with story time and other activities
included.  In addition to preschool and the summer activities, twice a month
we have a music instructor that comes to the house for 30 minutes of music
and fun.
            

RELIGIOUS PRACTICE

I do not include religious instruction or practices in my daily activities.  

We celebrate the following occasions: Christmas, Easter, Halloween,
Kwanzaa, Hanukah, Rosh Hashanah, Ramadan, and each child's birthday
             


TOILET TRAINING IS YOUR CHILD READY?
1. Expresses interest in coming into the bathroom with you to find out what
goes on there and perhaps even sits on the toilet herself.
2. Understands what the toilet is for and what it means to have a wet or dirty
diaper. If she also shows a preference for being clean and dry, fussing when
she's wet, pulling off a dirty diaper, or asking to be changed; all the better!
3. Knows the words for urinating and having a bowel movement (such as
"going potty" or whatever words your family chooses).
4. Can stay dry for at least two hours at a time.
5. Has regular bowel movements with soft, formed stools
6. Can and will follow simple directions, such as those for washing hands.
7. Can help pull pants up and down
8. Seems to recognize at least a few seconds ahead of time that she's about
to go, and can tell you before it happens. (Many youngsters will squat, leave
the room, or get "the look" before having a bowel movement.)
9. Is in a willing, receptive mood and isn't going through any major transitions
(like adjusting to a new sibling or school).
10. Demonstrates a desire for independence (for example, wants to be a "big
girl" and do things for herself) -- or, better yet, shows a specific desire to use
the toilet like mommy and daddy do!
If your child meets most of these criteria, she's ready to try. If not, wait a
month or two and reevaluate.
Potty-Training should begin at home over a long weekend or holiday.
Once you have had success at home for at least a week, your child may
begin wearing CLOTH training pants with rubber pants to cover them at child
care.
You must still provide me with 1-2 pull ups for nap or other extended periods.
Also you MUST provide me with at least 2 complete changes of clothing for
your child.
Under no circumstances will your child be allowed to potty-train in regular
underwear.
This is for sanitary reasons! Regular underwear cannot contain urine & feces
to prevent the spread of germs in my home & to other children in care & to my
family. Please cooperate with me on this matter.
Also, if you begin training, please notify me so that I can continue with all the
work you have accomplished.
Further, if within 2-3 weeks, your child shows no signs of progress, I reserve
the right to put your child back in diapers & try again in a few weeks.
When you feel your child is ready for toilet teaching, I ask that you begin this
teaching at home. I will follow through and encourage your child while in my
care. Please keep in mind that the activity level here can distract your child
from responding to an urge to use the potty, more so than at your home.
Therefore, I will continue to use diapers or pull-ups until your child can and
will announce that he/she must use the bathroom (not just at home, but here,
also) and can control his/her bladder and bowels for a few minutes beyond
that announcement. When the child has reached this point, training pants (5-
ply, not plain terry cloth) with plastic pants may be used.
During toilet learning parents will need to supply:
•        three complete changes of clothing (socks included)
•        5-ply training pants (6 pair per day)
•        plastic pants (6 pair per day)
•        diapers or pull-ups for naptime
When accidents are only happening once every other week, 5-ply training
pants will be used without plastic pants.
Do not bring your child in panties or underwear until he/she has naptime and
bedtime control established.
I also ask that during toilet learning, the child be dressed in "user-friendly"
clothing as much as possible. The best items are shorts and pants with elastic
waists, or dresses for girls. Try to avoid really tight clothing, pants with snaps
and zippers, and overalls as often as you can. Your child will want to help pull
pants, etc. up and down, plus clothing with too many "gadgets" makes it
harder to get the child on the potty in time
          

RENEWAL-
The Parent Agreement and forms will be renewed every year in September.
All previous forms will be obsolete.
                  
REFERRAL REWARD PROGRAM
As a way of saying thank you, I will offer a free week of childcare for one child
to each family that refers another family that enrolls full time. That family must
enroll their child(ren) and attend for 30 days in order for the complimentary
week to be awarded. This will allow you to have a part in choosing the
children your child(ren) will spend their days with.
                         
Termination

Termination policy:
You are required to give 2 weeks written notice when you decide to terminate
childcare.   Payment for the two weeks must accompany the termination
Notice.  The 2 weeks will be paid in full, regardless of whether your child is in
attendance or not.  I will give 2 weeks written notice of termination for which
full payment is due, whether your child is in attendance or not.
I reserve the right to give written notice of IMMEDIATE termination where
there are extreme circumstances that affect the well being of me or other
children in attendance.  Termination notices will not be accepted while
Provider or parent is on vacation.  The notice is to be handed to the provider I
will sign it and give you a copy at that time.  You must pay 2 weeks fees in lieu
of 2 weeks notice even if the child is not in attendance.  There is no 2 week
grace (adjustment ) period.

•        Failure of parent to pay
•        Failure to complete required forms
•        Lack of parental cooperation
•        Inability of program to meet the needs of the child
•        You as the parent need to give me a 2 week notice in writing.  (If you
don’t I will be charging you 2 weeks pay.  If you have an ending balance I will
take you to court for the remaining balance and 2 weeks pay.  (if there is a
ending balance then you are required to have it paid in full, if not I don’t have
to provide 2 weeks daycare but I still get paid fo2 weeks.
•        I will give you a 2 week notice unless the problem is a danger to me and
or the other children in my care.  

This Handbook will cover for a term of one (1) year from
May 23rd 1rd 2005 to May 23rd 2006.

Tuition may at my desecration increase periodically, but not during this term
of this contract.

This contract is binding.  Failure to cooperate with 1-2-3 Family Day Cares
handbook (policy) and contract will result in termination or court action, if
necessary.

I have read and I understand the rules for 1-2-3 Family Day Care and will
abide by the policy and contract.

The financial terms will be finalized upon signing of the parent-provider

1-2-3 FAMILY Day Care is licensed by the State of Wisconsin, Department of
Health and Family Services.  I am licensed to care for no more than eight
children at any one time.  I am inspected regularly to insure that I meet
licensing standards.  Smoking is not permitted on the premises of the center
during licensed child care hours.  My Husband Douglas does smoke on the
premises when the center is closed.

I will post the following items for your review: license certificate, results of the
last monitoring visit, any exceptions, a record of meals served.

Parents will receive a pamphlet, which is a summary of family child care
licensing regulations, "Your Guide to Licensed Child Care," as part of an
enrollment packet.  1-2-3 Family Day Care is covered by liability insurance for
my premises and/or for my operations.

1-2-3 Family Day Care does have pets on premise.  3 cats and lots of fish
they will be in the child care area.  .  All pets for which there is an effective
vaccine against rabies have been vaccinated.  The children will be closely
supervised when the animals are accessible to children to ensure that both
the children and the animals are protected from harm.  1-2-3 Family Day Care
as liability insurance that includes coverage for dogs/cats.  If your child has
allergies, please inform me.

Parents are welcome to visit my child care program at any time during the
hours of operation unless prohibited by a court order.  If so, I will need a copy
of the order.

Children will only be released to persons listed on the enrollment form.  If
anyone other than the child's parent or someone who is listed on the
enrollment form is to pick up a child(ren), I need to be notified in writing or by
a telephone call in advance.  The person picking the child(ren) up may need
to show a driver's license or other picture ID.

If the parent or other authorized person arrives to pick up a child and that
person appears to be intoxicated or under the influence of drugs all
reasonable steps will be taken to prevent the person from leaving with the
child, including offering to call a cab or another contact person.  While I
cannot legally withhold a child from the legal guardian I will not hesitate to call
the local authorities if I feel the child is in danger.

It is important that we communicate daily concerning the needs and interests
of your child.  If there are issues or concerns that need to be discussed
please work with me to arrange a convenient time to talk on the phone at
naptime or in the evening so we can give the issue the attention it deserves.  
To foster communication on a regular basis1-2-3 Family Day Care   provides
scheduled conferences/written newsletters/parent bulletin board/daily sheets.

To protect each family's confidentiality, 1-2-3 Family Day Care   will not share
information about a child or a child's family with anyone who is not authorized
to receive this information.

Parents must meet with me to discuss their child's specific needs and to
review program policies.  The following items must be completed and returned
to the center by the first day of attendance.

•        Form CFS-104, "Child Care School-Age Agreement" (if applicable)
•        Form CFS-56, "Child Care Center Transportation Permission" (if

•        First week's tuition
•        Parent/Provider agreement

I will inform you of any updates that are needed and give you 30 days to
submit the updated forms.

The following items must be completed and returned to me within 2 weeks
after your child starts attending.

•        Form CFS-60, "Child Health Report"
•        Form DPH-4192, "Day Care Immunization Record" or an electronic
record of your child's immunizations
•        Food program enrolment form

Children may be enrolled on a full-time basis (40hours per week or more), or
a part-time basis (less than 5 hours per day ).  No child may be regularly
enrolled for less than 10 hours or 2 days per week.  I do accept children for
drop-in care if prior enrollment arrangements have been made and space is
