Please read this ~Handbook~ thoroughly, as it covers very important policies and
procedures that not only governs our childcare contract agreement, but also pertains
to the care of your child. There may seem to be a lot of information, if you have any
questions regarding my policies, please does not hesitate to ask. We will go over this
book during your interview, but this copy is for you to reference back to. There will be
a yearly revision to this ~Handbook~ and the accompanying contract. All families will
sign a new contract each year. I reserve the right to make changes to the policies
and procedures, as I deem necessary. You will be notified, in writing, of any changes
that may occur.
Children will not be denied enrollment on the basis of sex, race, religion, national
origin, income level, physical or mental limitations or family configuration.




   
Dear Parents,
Welcome to1-2-3 Day Care. We are a small Family Child Care home. We are
different than a Day Care Center because we are limited to the number of children
that are enrolled. We only take up to eight children between the ages of newborn -
13 years at one time. By staying small we are able to provide the quality care and
nurturing atmosphere that your child deserves.
1-2-3 day care Play is not a preschool, we do offer an informal theme based
preschool program. Children participate in a full range of activities such as free play,
cooking, outside play, stories, dramatic play, creative art, crafts, music, and much
more.
Please read this handbook thoroughly, it covers very important policies and
procedures that pertain to the care of your child and family. Thank you for your
interest in 1-2-3 Day Care and please feel free to contact me with any questions,
comments or if you need clarification on any of the policies in this handbook.
Before you leave your child the first day, it is wise to make a visit with the child during
my regular day care hours (mornings are best, since quiet time is in the afternoon).
This way your child gets to see what happens in day care and it gives him/her a
chance to meet the other children and me. This visit is a good time to bring the
supplies that will be left at day care and the necessary paperwork for admission. If
you or your child is very uneasy about day care, at least two visits are recommended:
the first during my off hours when your child will not be overwhelmed by the other
children and the noise, and will have all the toys available to him/herself. During this
time I will be able to give your child undivided attention. The second visit could be
during regular day care hours. I can also give you some ideas on what you can do to
make the transition as easy as possible for you and your child.

Please allow me to tell you a little about myself. My name is Kris Steiner and I am a
Family Child Care Provider.  (not a baby sitter) I don’t sit on Babies.
I am Married to a wonderful man named Douglas Steiner he has supported my and
my dream to own a childcare center for 13 years now! He is the repair man
construction worker, the carpet layer, fixes all broken appliances (for daycare is hard
on all) the flower and garden planter law mower and snow shovel just to name a few!  
We have 4 awesome kids! Christopher a 7th grader he is a 2nd degree black belt in
karate,  Mariah a 2nd grader is in modeling and dance and karate,  Hunter a pre
schooler he is also in karate and Fisher a 2 yr old.  
I understand how difficult it is for parents to find good quality child care anymore. I
feel I have the love, knowledge, experience and qualities needed to provide a warm,
loving environment in which children can learn and grow. I believe that children are
the most important part of our future, and I will work with you to help establish the
solid foundation children need to feel secure and to provide stability for their future.
I take my profession as a child care provider, not a "baby sitter", very seriously. I
have completed many hours of early childhood education and will obtain as much
continuing education as possible in order to constantly improve my knowledge and
skills. I have also received Red Cross certification in Infant, Child and Adult CPR and
First Aid.

Although family child care is a form of self employment, unlike other businesses,
there is no room for physical growth or expansion due to licensing regulations. I am
not guaranteed any benefits that most employees take for granted such as: health
insurance, personal days, workmen's compensation, unemployment insurance,
retirement/pension plan or bonuses.

1-2-3 FAMILY Day Care is licensed by the State of Wisconsin, Department of Health
and Family Services.  I am licensed to care for no more than eight children at any
one time.  I am inspected regularly to insure that I meet licensing standards.  
Smoking is not permitted on the premises of the center during licensed child care
hours.  My Husband Douglas does smoke on the premises when day care is closed.
POSTING
I will post the following items for your review: license certificate, results of the last
monitoring visit, any exceptions, a record of meals served.

Parents will receive a pamphlet, which is a summary of family child care licensing
regulations, "Your Guide to Licensed Child Care," as part of an enrollment packet.  1-
2-3 Family Day Care is covered by liability insurance for my premises and/or for my
operations.

1-2-3 Family Day Care does have pets on premise.  3 cats and lots of fish they will
be in the child care area.  .  All pets for which there is an effective vaccine against
rabies have been vaccinated.  The children will be closely supervised when the
animals are accessible to children to ensure that both the children and the animals
are protected from harm.  1-2-3 Family Day Care as liability insurance that includes
coverage for dogs/cats.  If your child has allergies, please inform me.

Parents are welcome to visit my child care program at any time during the hours of
operation unless prohibited by a court order.  If so, I will need a copy of the order.

Children will only be released to persons listed on the enrollment form.  If anyone
other than the child's parent or someone who is listed on the enrollment form is to
pick up a child(ren), I need to be notified in writing or by a telephone call in advance.  
The person picking the child(ren) up may need to show a driver's license or other
picture ID.

If the parent or other authorized person arrives to pick up a child and that person
appears to be intoxicated or under the influence of drugs all reasonable steps will be
taken to prevent the person from leaving with the child, including offering to call a
cab or another contact person.  While I cannot legally withhold a child from the legal
guardian I will not hesitate to call the local authorities if I feel the child is in danger.

It is important that we communicate daily concerning the needs and interests of your
child.  If there are issues or concerns that need to be discussed please work with me
to arrange a convenient time to talk on the phone at naptime or in the evening so we
can give the issue the attention it deserves.  To foster communication on a regular
basis1-2-3 Family Day Care   provides scheduled conferences/written
newsletters/parent bulletin board/daily sheets. To protect each family's
confidentiality, 1-2-3 Family Day Care   will not share information about a child or a
child's family with anyone who is not authorized

and crafts, most field trips, travel to school and back etc…

Parents must meet with me to discuss their child's specific needs and to review
program policies.  The following items must be completed and returned to the center
by the first day of attendance.

I have my State of Wisconsin licensing certificate and My Grant County certified
childcare certificate on the bulletin board in the entryway. I will also post all
monitoring visit reports there if there is any questions feel free to ask!
Children may be enrolled on a full-time basis (40hours per week or more), or a part-
time basis (less than 5 hours per day ).If I have the Space available.  I do accept
children for drop-in care if prior enrollment arrangements have been made and

I believe that children receive the best possible care when there are eight (8) or
fewer children under the age of 12 per adult.  Except with prior agreement from all
parents using my services, I will not care for more then two infant (under 12 months)
at a time without my helper here. (Look at tuitions rate page for more information
37)   When it comes to filling vacancies or increasing the number of children I care
for (a maximum of 8) I will always give preferential consideration to the families I
currently serve.   I would currently welcome the opportunity to care for new siblings
as they arrive.

Children need to feel that they are getting quality attention from the adults in their
lives, and they also benefit from lots of one-on-one time.  Therefore, for the
children's' benefit and mine, I will attempt to keep the group as small as possible.  
From time to time in order to accommodate the families that I serve, it may be
necessary to accept one or two extra children for a day or two, such as in the case of
a school closing due to weather conditions.  On these days I reserve the right to
accept older children for temporary care decide additional fee.  To the extent
possible, I will notify all parents in advance of such days, so that you can decide if
you wish to keep you child in care that day.  Be assured that I will never accept more
children for care then I believe would be safe and in the best interest of all the
families that I serve.  I anticipate that this will be a rare event and not a routine
situation.

Occasionally I will ask my mother to help watch the children in my home or during a
prearranged field-trip.  You will have the opportunity to meet her, and I will always
attempt to let you know in advance when she will be assisting me.  I will also welcome
your assistance during special field trips or other special fun events.  I realize that
you would not be hiring me for care if you regularly had time to spend daytime hours
at home, so this time would be purely voluntary.  Of course you are always welcome
to drop by anytime during regular business hours.




Absences       
      Initial _____________        

There will be no refunds or adjustments made to your childcare fees for your time
missed due to illness, holidays, vacations (yours), or days off.  A place has been
reserved for your child that cannot be filled on a short-term basis.  Although I will
make every attempt to be available each day, there will be occasions when I or
another family member is ill and I am unable to provide care. You will be notified as
soon as possible of any personal or family illness, funeral, or emergency. It will be
your responsibility to obtain alternate care on those days.  Please notify me no later
than 9 a.m. if you will not be bringing your child or will be arriving late.  More than 3
consecutive absences without a call may be considered a termination by Parent with
all fees due & payable.

Administer Medications  
Initial _____________

Since I am not a licensed medical professional, I will not administer any type of
medication including prescription and over-the-counter medications, without a signed
consent form (see attached).  This form can be filled out when you drop off your child
and must include:

•        The child's name, age, and weight
•        The name and the phone # of your child's doctor
•        The name of the medication
•        The dosage amount and frequency
•        Instructions for proper storage and preparation of the medication such as
should it be refrigerated, shaken, mixed with water etc...)
•        Reasons for needing the medication (so parents can be alerted to contagious
illness)

Also, the medication must be clearly-labeled in child proof container.  I would prefer
you also bring the medication in a small sack or baggy labeled with the child's name.
This is to avoid any possible confusion.
Although I am trained in infant and toddler CPR, basic first aid, and recognition of
communicable childhood diseases, I do not pretend to be a doctor, and will not under
any circumstances provide any medications, including vitamins.  You must provide
any and all medications for your child.

Arrivals & Departures

When children leave for a field trip or school run  (leave the daycare for any reason
the back sheet of this policy will be used!)
Children are to arrive clean, dressed and fed (unless arriving just before a meal
time).  It is normal for some children to have difficulty separating from parents, or cry
when being dropped off.  Please be very brief (no more than 5 minutes) during drop-
off times; the longer you prolong the departure, the harder it gets.  A smile, cheerful
good-bye kiss, and a reassuring word that you will be back are all that is needed.  In
my experience, most children are nearly always quick to get involved in play or
activities as soon as parents are gone.

Please be brief at pick-up times also.  This is a time of testing when two authority
figures are present (the parent & provider), and all the children will test to see if the
rules still apply.  Typically pick-up times need to be kept brief.  During arrivals &
departures, I expect you to back up my House Rules (posted below), but if you do
not.  I will remind your child that their behavior is inappropriate and take action to
correct, if necessary.  Please be in control of your child during these times.  Do not
allow your child to run out to the car while you are still inside!  The safety Rule is “No
one goes outside without an adult with them.”

Sometimes, at pick-up time, the children may get a little over-excited and/or rowdy
because someone else (parents) are here. Please help show your child that you
respect me and my rules by reminding them (and allowing me to remind them, if
needed) that the rules still apply whether or not someone else is here.  Or whether or
not you are here some children act out to get their parents attention and that tends
to get the whole group started. (There is a $1.00 per minute early drop off, and late
pick up charge. )  It is due on the next schedules day of childcare.  

Tips for Smooth Transitions at Drop-Off and Pick-Up

Transition times can be stressful for some children. In the morning they may not want
to say good-bye to you and at pick up time they may not want to go because they are
involved with a fun activity. This is a natural occurrence and may vary with your child’
s age and developmental stage.
To ease the situation we encourage parents to take a minute to help their child feel
comfortable and to adjust by offering positive statements. A parent who shows
reluctance to depart only makes the transition harder for their child. Although parents
should never sneak out without saying "good-bye" to their child, a brief good-bye
usually works best. Crying will usually stop a few minutes after you depart.  Children
of all ages adjust to transitions differently. When you arrive to pick up your child, we
will let you know briefly how their day went, any special events or accomplishments,
eating, sleeping and bathroom habits because the children, as well as the provider
and/or assistants are all anxious to end our long day care day.  If you have any
concerns and/or issues, I ask that you call and arrange a time to discuss the matter.  
Some concerns and issues should not be discussed in the presence of the child,
children and/or other parents.

The provider will recognize each child’s birthday by announcing it on the chalkboard
as well as singing the birthday song with all the children. Parents are welcome to
bring a special treat (cupcakes, brownies, etc.) as long as you bring enough for
everyone.

Child abuse    
Initial _____________

Unfortunately, this is a topic that must be addressed in today's society.  It goes
without saying I will not knowingly allow any form of sexual, physical, or emotional
abuse of any child in my care at any time. This includes the child when in their own
home.  On the surface, this may sound like an invasion of privacy, but I cannot
ethically turn a blind eye toward such abuse.

I have been trained in the recognition of all forms of child abuse, and will do
everything in my power to prevent any instances from occurring.  If I notice any signs
of abuse at any time, I will alert the child's parent(s) immediately both verbally and in
writing, along with a full description of the symptoms. (Bruising, sores, sudden
extreme behavior changes, exedra.)  I will document the same information for myself,
and if necessary, alert the proper authorities after first attempting to handle the
situation discreetly.

You may get notes about your child having bruises and scrapes without needing to
fear that I suspect you of abusing your child.  Be assured, if I think your child has
been abused, I will let you know!  The notes that I send home are just to alert you in
general of any instances that may come up, and also to protect myself from mistaken
accusation of abuse.  I would prefer not to have to deal with this issue, but I think it is
best that we handle it openly and honestly to avoid any possible misunderstandings.  
We both want what is best for our children. Please feel free to discuss this or any
other issues with me at any time.

CONFIDENTIALITY
Information about you and your child is considered confidential. This means that
Information, including whether a child is enrolled or not, will not be shared outside of
our daycare staff without a written release from you. Staff is trained in issues related
to confidentiality and maintains confidentiality at all times. 123 Family Daycare staff
will not share pertinent information with anyone but me Kris Steiner. Likewise,
information pertaining to the performance issues of any staff members at 123 Family
daycare is confidential. As a parent, if you have a concern about staff performance,
please refer to section Page 39-40 Resolution of Parent Concerns on these
procedures


CLEANLINESS/HYGIENE     Initial _____________
I do my best to maintain strict cleanliness and hygiene standards. Children's hands
are washed before and after meals and after toileting. We use paper towels for
drying hands, so children do not have to use the same towel.
Young toddlers sleep in separate mats, with clean sheets used only by them,
washable nap mats are used.  All mats are cleaned twice a week.
Children use either paper plates/bowls & utensils that are discarded after each use
or plastic plates/bowls and utensils that are washed in the dishwasher after each use.
High chairs and table areas are cleaned thoroughly after each meal.
The provider’s hands are washed after every diaper change and as needed
throughout the day. The area is sanitized and the diaper is wrapped in a diaper
disposal bag and thrown away. Toys and playroom items are washed and sanitized
on a monthly basis.


Clock In/Out:

All children must be clocked in and out.  There is a Time clock and time cards in a file
in the entryway on the cubbies. Each child has their own time cards.  Please clock
them in and out with the current time and your initials.  If they are picked up or
dropped of by a bus I will sign them in or out at that time.  Use the "back of the card
for notes to me!
“Column to make note of important information (I.E.: a morning dose of medication,
early pick up time, dr. appointment, new work phone number, etc.).  This is done for
tax and legal purposes so that I can show that your child was here during a certain
time frame. Failure to clock your child in and out is a violation of Wisconsin daycare
licensing rules so I will fine you $1.00 per time I have to do it! So let’s just work
together on this one!

COMMUNICATION WITH PARENTS      
Initial _____________

Keeping close communication with my families is a priority!  Because we may not
always have the time to talk about how and what your child is doing, I have
established the following communication system to help us keep in touch at all times:

~Daily Reports~
Each child will have a daily written report detailing their activities and routines.  
Infants will have a journal (if two-way communication is maintained via the journal)
detailing their day; again, two-way communication between caregiver and parents is
strongly suggested by way of the journal.

~Entryway Posting~
I will post information regarding upcoming events, reminds, on the wipe off board
below the time clock! My licensing and certification certificate and recent results of
last monitoring visit will be posed here (to the right of the time clock!)

~Parent’s Board~
In the kitchen, there is a Parent’s Bulletin Board which contains our monthly calendar,
curriculum information, community information and other fun tidbits.

~Mail boxes~
Each child will have a file accessible to parents.  This is where receipts and notes to
parents will be placed.  

~Parent Conference~
occasionally parents will be invited to meet and discuss their child’s development and
concerns.  This may be initiated by either the parent or myself.

~Calendars and Newsletters~
A monthly calendar and a monthly newsletter will be distributed to keep you informed
about learning themes, special dates and topics of interest.  The URL for the

~Evaluation Forms~
I encourage you to give me feedback about my program through conferences, daily
conversations or surveys in a formal evaluation.  Written surveys are given to each
parent twice a year.  Please be honest as I will use this information to better my
program.

PARENT PARTICIPATION

Occasionally I will invite parents to some special events each year such as their child’
s birthday party or special holiday parties.  Additionally, you may participate by
helping with a field trip or in any way you like.  Of course you are always welcome to
visit at any time during the daycare day!

Custodial Agreements:     Initial _____________

Clients that have Custodial agreements would be wise to provide a copy to me for
safety reasons.  All arrangements are different and for the protection of your child,
your family, and my daycare this will assist if a parent shows up to take a child that
has no right to have that child.   I can’t release a child to another parent without
permission from the client that I hold the contract with unless I have a signed consent
form. CUSTODY ISSUES AND UNAUTHORIZED PERSONS:
If only one parent has custody of the child(ren) and the other is not authorized to
pickup the child, the parent must instruct the provider of this fact, and must provide a
copy of the court order confirming that one parent does not have (or has limited)
visitation rights. If there is any change in the legal custody of the child, the parent
must provide written documentation confirming the change. The custodial parent will
be notified immediately if an unauthorized (someone not on the forms or a non-
custodial parent) person arrives to pick up the child.

DAILY ACTIVITIES/SCHEDULE

The main goal of this childcare is to have fun, improve social skills, and encourage
creative expression. I will utilize a variety of activities to accomplish this goal. Free-
play, reading, arts and crafts, music/singing, dancing, dramatic play/pretend, puzzles,
and educational TV/videos are just some of the activities we will be doing.
Creativity plays an important role in my daycare program. We do not follow a strict
regiment of scheduled "class" time, but grab every opportunity to turn playtime into a
learning experience. In order for children to be creative, they must be free to make
choices, and then receive the encouragement to carry them out. Open-ended play
materials will be provided that will enhance the individualistic abilities of the children.
Young children still learn best from participating in and observing the environment

Young children and toddlers enjoy a structured schedule that allows for flexibility. A
schedule helps the day to flow more smoothly, allows the child to anticipate coming
events, and aids in achieving a variety of goals. We will adhere to our schedule to
the best of our ability, keeping in mind that anything can happen when children are
involved. There will be times when we have to make adjustments to the schedule. We
appreciate families considering our schedule when picking up or dropping off their
children.
TV time is closely monitored and is limited to PBS, Nick Jr. and Playhouse Disney.
Movies are limited to only educational videos.
Music helps to develop young brains and will play a strong role in day-to-day
activities. We may have special music activities and may also play music during other
activities throughout the day.

I will plan activities and provide children with a variety of experiences.  Some of the
activities will include:

      Language development: Books, music, story time, fingerplays, flannel board
stories

      Large muscle skills: Balls, hula hoops, bean bags, swinging, outdoor play

      Small muscle skills: Art and crafts activities, stringing beads, Legos, pegboards,
blocks

      Creative expression: Dramatic play, puppets, music and musical instruments, art
activities

      Self-help skills: Assist with mealtime preparation, dress self for outdoor, help with
jobs in the center

Play is the major component of our program.  Enough time, materials and space will
be provided for children to actively explore the world around them.  Children will have
an opportunity to use a variety of art materials, manipulative and housekeeping
equipment.  Children, including infants and toddlers, will go outdoors daily when
weather permits.  The children will be kept indoors if the the temperature is below 0
degrees including wind chill; raining or storming.

I will allow children to watch television including Videos or DVDs.  Children may not
bring videos or DVDs from home.  The children will be allowed to watch television
before naptime, during the early morning and late afternoon or on special
occassions.  Children are not required to watch television and other activities will be
available during that time for children to use.

School age children will have a quiet place to study or relax, access to appropriate
materials and activities, and will have ample time for large muscle activities and to
participate in food preparation.

Children under two years of age will have a flexible schedule, which reflects the
child's individual needs.  They will be given individual attention including lots of time
for talking.  The body position of non-mobile infants and their location in the center
will be changed frequently.  I will provide safe, open spaces for children whom are
creeping and crawling.  Infants and toddlers will be encouraged to play with a wide
variety of safe toys and objects.


Discipline Policy

I do not believe in spanking, slapping, yelling, or hitting of any kind. This type of
discipline will not be used in my home, regardless of your practices in your home.
Instead, I use the following discipline methods to handle any dangerous or harmful
offense:
Children's behavior will be guided by setting clear limits or rules for children.  I will talk
with children about expected behaviors and model those behaviors consistently for
them.  I will state positively what children can do, using specific terms; e.g. "you need
to walk" rather than "don't run").  Undesirable behavior will be redirected to another
activity.  Children will be given a wide variety of age-appropriate activities to choose
from and will be given the attention they need before they demand it.  Behavior
management will be for the purpose of helping children develop self-control, self-
esteem and respect for the rights of others.

I do use "time-outs" to deal with unacceptable behavior.  A "time-out" or "take a
break" may be used when other techniques have not been successful.  A time-out will
be used to remove a child from a situation that has gotten out of control before a
child could hurt himself or others.  Time-outs will never exceed five minutes and may
not be used with children under three years of age.  When used, the time-out will
immediately follow the behavior.  I will stay with the child and talk about what behavior
was unacceptable, and what else he/she might have done or said instead.  Rather
than use a specific time-out chair or corner, I will have the child "take a break" near
the others, so the emphasis is on relax/cool down, rather than isolation and
punishment.  The child will be praised after completing the time-out, and will be
helped to rejoin the group.

I recognize that no single technique will work with children every time.  If a child
exhibits unacceptable behavior, I will request a conference with parents to consider
how to deal with the behavior.  If the behavior continues, the next steps may include
referrals to appropriate community resources, and/or discharge of the child from care.


In accordance with "Wisconsin Rules for Licensing Family Day Care" actions that are
aversive, cruel, humiliating, and actions that may be psychologically, emotionally or
physically painful, discomforting, dangerous or potentially injurious are prohibited.  
Prohibited actions include spanking, hitting, pinching, shaking, slapping, twisting, or
inflicting any other form of corporal punishment on the child; verbal abuse threats or
derogatory remarks about the child or the child's family; physical restraint, binding or
tying the child to restrict the child's movement or enclosing the child in a confined
space such as a closet, locked room, box or similar cubicle; withholding or forcing
meals, snacks or naps; punishing a child for lapses in toilet training.  These forms of
punishment will never be used, even at a parent's request.
For children under the age of 18 months:

I find it most effective to remove the child from the situation, and redirect their
attention elsewhere. Although I will most likely explain to the child that the offending
behavior was inappropriate, children of this age are rarely able to fully understand
what they have done.  Fortunately, their attention span is also shorter than the older
children, so simply showing them a different toy or activity usually does the job.
For children of about age 2 and over:

I will alert the child to the offense by saying firmly "no", explaining why the behavior is
unacceptable, and offering the child the choice to behave.  If the child chooses not to
behave appropriately, I will first explain to the child why their behavior is not
acceptable, and then I will place the child in a chair for "time out” about 1 minute per
year of age. At the end of the time out, I will ask the child if he or she understands
why they had a time-out , ask if they understand why they shouldn't do that behavior,
then ask if they are ready to play nicely again.  This method not only stops the
offending behavior, but also teaches the child consequences, responsibility, and
empathy in a positive manner. Dangerous and hurtful offenses including hitting,
biting, kicking, pushing, hair pulling, throwing objects at someone, climbing on an
inappropriate structure, willful destruction of property, tantrum throwing, and not
following reasonable requests.  These relatively minor offenses will be handles as
described above, depending upon the child's age and developmental stage.

To a point, kids will be kids- but only as long as the emotional and physical safety of
all the children in my care are protected.  In rare instances that I feel any child in my
care has serious discipline problem; I will request a conference with the parent(s).  If
an understanding cannot be reached, I reserve the right to terminate our contract
with as much notice as possible in order to guarantee the comfort and the safety of
the other children in my care.



Emergency Procedure    
Initial _____________

In case of an emergency that would require an evacuation, children will be evacuated
through the nearest exit.  The attendance form and list of phone numbers for parents
and emergency contacts will be taken out to assure that all children are accounted
for and all families notified.  Children will be assembled at 309 Pearl St.

In the event of a tornado warning, the children will be taken to location.  Blankets, a
portable radio and flashlight, with extra batteries for both, are kept in the tornado
shelter area at all times.  The attendance form and emergency contact information
will be brought along.
In the event of a lost child, I will check all areas of the center.  If the child cannot be
found, the child's parents and/or emergency contact and the police will be
immediately notified.
If a child who is scheduled to arrive at the center, does not arrive within 1 hour of the
specified time on the written agreement signed by the parent, I will call the parents
and let them know the child has not arrived. The call will be documented.
If the center should lose the use of heat, water or electricity before the center opens;
or while children are in attendance, I will call parents and ask them to pick up the
children or I will call parents by 6 AM and ask them not to bring the child that day.
If we are unable to re-enter the building after a necessary evacuation, I will take the
children to  Casey’s Gas Station and contact parents as soon as possible.
There will be a posted drill and evacuation plan for the following: tornado, fire and
serious injury or illness. We will have practice drills for the above according to the
State Licensing Requirements. In the case of a power outage due to inclement
weather, we will contact the parents so we can decide together what would be the
best plan of action for your child.For life-threatening emergencies, we will call 911 for
assistance and accompany the sick child to the hospital by ambulance, leaving the
other children with a competent adult caregiver.  (Debra Fortner) For non-life
threatening emergencies, parents or other listed adults will be phoned to take the
child to the doctor or emergency room. In a event that I have to leave I will

Fire and Tornado evacuation plans will be practiced monthly.
      
Orientation Plan for emergency Back-up Substitute Provider

Each employee, Volunteer, and/or Substitute will receive an orientation before
beginning working here at 1-2-3 Family Day Care.  The orientation will be
documented on the Staff Orientation Checklist. This orientation will include
1)        The Names and ages of all children in care.
2)        The current arrival and departure information for each child enrolled including
the names of the people authorized to pick up the child(ren)
3)        The location of children’s emergency contact information (files)
4)        Specific information relating to the children’s specific health care needs
including medications, disabilities or special health conditions.
5)        SIDS risk reduction procedures (only if licensed to care for children under 1
year of age).
6)        An over view of daily schedule and any o schedule.
7)        Emergency procedure.
8)        The procedure for reporting suspected abuse The plan for evacuating
sleeping children  (only of care is provided between 9:00 p.m. and 5:00 a.m.
The back up providers will receive a verbal orientation each time before being left
alone with the children.
If there is a minor injury  
I will wash the wound with soap and water and apply triple antibiotic ointment and a
band aid
If there is a Major injury
I will call 911 and I will call the parents and the person on the emergency contact list
and explain and ginger Schafer will be called in for back up care in event I have to
leave the premises with the child(ren)

Medical log book
I will document in my log book any changes in the children attitude or discipline
issues and if anything is not normal.  If there is family issues it will be wrote in the
book it is like my diary! If a child is bit by a bee bug or another child it will go int here
if I suspect child abuse it is there too! I have to do this for the safety of all children in
my care including my own!


Field Trips:

Field trips (2-3 times a month) we twill be traveling to local libraries, post office, fire
station, police dept, bakery, newspaper shop, other factories, hospitals, post office
ect…  I feel no matter the Childs age they learn from hands on.  I feel children can
and will learn so much more with the experience right in front of them and letting them
reach the learning opportunity to learn Parents are always welcome to accompany us
on trips.  And sometimes I will ask for a helper or two for these types of trips in my
newsletters so feel free to volunteer or have a friend or relative help out if you would
like.

Signing this contract implies that you are allowing me to transport your child in the
proper restraints in my vehicle for field trips, various outings, and school drop off and
pick ups.

In the event of a funeral, The Provider is paid two days for a family funeral.   The
provider will be attending 5 paid professional training days per year.    Parents will be
given at least a two week notice before the scheduled training day.   Training days
are beneficial for meeting your children’s needs while in my care and a must to
provide better service for you and your children.   The provider will notify you as
soon as possible if I will be closed due to illness or a funeral. Parents may wish to
secure their own back-up provider for all of these situations.

FUNERALS /PROFESSIONAL TRAINING

Holding fee
On certain occasions a family may need to take their child out of the center but would
like for us to hold a spot for their child. A holding fee of $75.00 per week, used in 4-
week increments up to 8 weeks can be paid to retain your child’s placement.   
Maximum of 12 weeks holding fee per calendar year.  Weeks may not be carried over
into the next year (Dec. into Jan.)  NO holding fee is allowed for children from birth
through age 2.  A 2-week written notice is required in order to start the holding fee  If
notice is not given, regular tuition rate will be charged.
Each parent has the option of not paying the holding fee. However, be aware that if
you do not pay the holding fee, we cannot guarantee placement within daycare when
you wish to return.

falls on a week day I will be closed for he day! It is a paid holiday for me so your
weekly tuition will not change!

Children and families are welcome in areas of my home that are used for childcare
purposes. My family and I need some privacy and separation from the childcare.
Please respect this. It can be difficult for my family to share their home/wife/mother,
so intimately, with other children/families, for so many hours each day. It is important
for them to feel that they have their "own" space. My children are expected to keep
any toys/objects that they are not prepared to share, in their rooms, away from the
other children.
1. I am not responsible for lost or broken items that children choose to bring from
home.
2. Please limit toys and books from home. Please NO small things that the little ones
could put in their mouths (including coins).
3. All children have a rest time in the afternoon.
4. There is no physical punishment in my childcare. A child may be removed from the
play group for a cooling off period of 3-5 minutes.
5. I can not accept sick children into my care. If your child becomes ill while in my
care, you will be notified and your child will need to be picked up as soon as possible.
6. Due to the risk of injury while jumping on a trampoline, the Wisconsin Department
of Human Services, Division of Child Care Regulations prohibit the use of a
trampoline by children in a home daycare facility.  While your child is under my care
he/she will not be permitted to use this equipment.

7. If I must be away from the daycare, i.e. Doctor’s appointment, for a short period,
my husband Doug, will provide substitute care.

If your child will not attend on a regularly scheduled day please let me know by 8  A.M.


I will transport children who attend school at Boscobel Elementary to and from the
center in the morning and after school.
If parents wish to allow a school-age child to leave or arrive at the center unescorted,
they must provide written authorization for this activity.  School-age children who
leave the center unescorted must be traveling to home, school or another activity
where adult supervision is present.  

Although I am not a trained professional, I have been educated in recognition of
infectious childhood diseases and will attempt to alert you at the first sign of any
illness.
In order to protect the health of all children in my care, I will call you at work and
request that you pick up your child within one hour if he or she exhibits and of the
following symptoms:

•        Fever over 100 degrees for more then 1 hour
•        Excessive diarrhea for 2 or more consecutive diapers or toileting
•        Vomiting in excess of typical infant spit up
•        Conjunctivitis (pink eye)
•        Consistent complaints or ear or stomach pains
•        Bleeding other then minor cuts and scrapes
•        Excessive greenish nasal discharge, indicating possible infection
•        Head lice
ILLNESS AND SICK CARE
The health and well being of all of the children here are of utmost importance to me.
It is for the protection of the children that I must adherence to my Health Policy.
Please read it carefully. If you have any questions or doubts, regarding the
statements it contains please discuss them with me now. Do not wait until your child is
sick to think about what would be best for all concerned. At that point you may be
making the decisions based on emotion instead of logic. Even with all of our
precautions children do get sick and or hurt. Due to my concern for all of the children
enrolled in my child care there are certain guidelines that I require my clients to
observe. In some cases, if your child needs to be seen by a doctor, you may be
required to submit a signed report from your doctor before your child can return to
child care. I will furnish you with a form or a form can be obtained from your doctor.
This is to ensure that a child does not return to child care when he or she may be in
danger of exposing someone else to an illness. Some contagious illnesses are no
longer contagious after the child has been on medication for 24 hours.

There are a number of immunizations required by law before your child may attend
child care. Upon application for enrollment you may be asked to bring your child’s
immunization record. You will be informed of any immunizations that will be needed
before your child starts child care.

Children with minor illness may attend child care at the providers discretion. It is
important to realize that if a child is unable to participate in the normal routine or
needs more care than I can provide without neglecting the others in my care, that
child must stay home. I know we both agree there are times a child needs to be with
the parent for both physical and emotional comfort. There are also some illnesses
that by law exclude the child from attending child care. Some of those illnesses are
but not limited to:

Infectious Conjunctivitis
Infectious Diarrhea
Impetigo
Chicken Pox
Hepatitis A
Scarlet Fever
Scabies
Lice
Ringworm
Strepthroat

I would be happy to discuss the details of any of these infections with you at your
request. When a child has certain symptoms, he should be kept home or in a special
setting. Some of these are:
FEVER
A fever is a sign that the body is fighting some problem. The importance of a raised
temperature depends on what is causing the fever. For example, if nothing else is
wrong and the child had a DTP shot, the fever is not cause for the child to stay home
unless it directly affects his or her ability to handle normal daily activities.

There are certain times when a fever means a child should stay home. These
include: An infant up to 4 months with a temp of 100 degrees or higher. A child 4-24
months with a temp of 101 degrees or higher. A child over 24 months with a temp of
101 degrees or higher. In the event of this type of temperature, the child should not
return until the temperature has been gone for 24 hours without the aid of fever
reducing medications such as Tylenol. If your child awakes with a high temp and you
administer a fever reducer, this generally only lasts a few hours and I will have to call
you when you arrive at work to come pick up your child. In the meantime the other
children have possibly been exposed to an undiagnosed illness.

VOMITING OR UPSET STOMACH
A child who has been vomiting can easily spread germs through vomit. The child may
also need my help. The added time to clean up after the instance takes my attention
away from the other children. If your child vomits while at child care, you will be
expected to come immediately to remove your child from child care. If you are not
able to come as soon as you are called, please arrange for someone else to come
pick up your child. The child must stay home until 24 hours has passed with no
vomiting episodes.

DIARRHEA
When a child has a single loose stool, he or she does not need to be at home.
However, if a child has very runny stools that cannot be contained in a diaper, or the
child cannot reach the toilet in time, the stool may contaminate the child care setting
and this child must remain home. Sometimes a child may get diarrhea from antibiotics
or eating something different or to much of something. If they feel well and do not
need extra attention and are able to participate in normal daily activities they do not
need to stay home. Children with diarrhea, who look or feel sick, or have a fever
along with their diarrhea need close attention. They should stay home until 24 hours
have passed with only one bowel movement.

RUNNY NOSE
Children with constant runny noses that are not caused by allergies may spread
germs everywhere. They may wipe their noses on their hands, then rub them on their
own and others clothing and on surfaces and toys. This is a difficult one to call.
Please keep in mind how you would feel if another child’s parent brought their child to
care and exposed your healthy child. These instances will be handled on a case by
case basis. Your cooperation will be greatly appreciated. Other symptoms of illness
that will cause some concern are: gray or white stool, infected skin patches, difficult
or rapid breathing, pink eye, severe itching, sore throat, severe coughing, yellowish
skin or eyes, spots or rashes, dark urine, headache/stiff neck, and unusual behavior.

Some of the features that help insure your child’s health are:
*NO SMOKING ON THE PREMISES.
*Current immunizations are required
*Good hygiene is stressed at all times
*Infants are held while drinking from a bottle. Therefore children are not allowed to
carry around or have a bottle in their beds.
*Favorite blankies or sleeping toys may be brought but kept for rest time.
*Children do not bring food or drink to child care. (the only exceptions are pre-
arranged treats )
*Children do not bring toys from home.
*Children are prevented from sharing cups, utensils and food
*Every attempt is made to keep toys and play area’s sanitized.
* Napping is done on individual beds or mats.
*Food preparation is done in a safe and hygienic manner.
MEDICATIONS
If your child is on medication and it must be administered while at child care, the
medicine must be in the original container and labeled with the childs name, doctor,
name of medication, dosage and when it is to be taken. I will also have a form for you
to sign giving me permission to administer the medication to your child. Medication
will be given at the time or with the meal you specify and a written record kept.

DIAPERING
The diapering surface is waterproof and free of cracks. A washable cover is used
when changing diapers. Soiled diapers and wipes are immediately thrown away.
Hands of the child and caregiver are washed thoroughly and dried with a paper
towel. Diapers are changed as needed and are checked often. Creams, ointments
and powders are not routinely used. On occasions that these products are needed
or requested by the parent it will be discussed with parents before applying.


In event that your child is sent home due to one of the above conditions he or she is
not aloud to return until accompanied by a signed by the dr.  This is necessary to
prevent the unnecessary infection of the other children in my care.  Although it may
seem inconvenient when your child is sent home, you will appreciate knowing your
child's exposure is minimized when the other children become ill.

I will not send a child home with a common cold.  This sort of illness is going to
accrue among children, and unfortunately cannot be totally avoided.  You must
expect that anytime a new child is introduced to the groups, cold are likely to accrue.  
I will do my best to prevent the spread of such illness through hygiene and house
keeping, but totally avoidance of this sort of thing is impossible.  

Likewise, I will not close my daycare if I have a mild cold. I will, however notify you as
soon as symptoms appear in myself or any other children, so that you can decide
whether or not to have your child in care that day.

I will follow procedures on personal cleanliness and communicable diseases with
licensing rules and the guidelines for exclusion of children from child care as adapted
from the Division of Public Health.

In the event that I or a household resident is ill, I will notify the parents of all regularly
scheduled children no later than 6a.m. .that the center will be closed.

All immunizations must be up to date. Any child found not up to date will have 24
hours notice to receive required immunizations. The provider can refuse to care for
any child(ren) if required immunizations have not been received.

Indoor Play: We have a large playroom area designated for the children to play in.
This room has plenty of toys and floor space for each child to play together with the
other children or by themselves if they choose. With the exception of meals and
outside play, the children will spend most of their day in the playroom as it is easier to
monitor and safer for the children to be in one area of the home. The children will be
expected to clean up as needed throughout the day.
Outdoor play: We will be playing outside everyday that weather permits. Please make
sure that your child is appropriately dressed for outdoors play at all times. During the
colder months, our outdoor play will probably range from 15-30 minutes, and in the
warmer months, we are normally outside for 1-2 hours each day. Outside activity and
exercises for children are important for stimulation and exploration. They will only be
permitted to play outside when the weather is dry. When the weather is over

Since 1992 the American academy of pediatrics has recommended that infants sleep
on their backs to reduce their risk of sudden infant death syndrome (SIDS). But some
families still place infants on their stomachs to sleep. In order to provide the best care
for your infant the daycare does the following:
•        Infants under 12 months will be placed on their backs on a firm, tight fitting
mattress for sleep (in a portable  crib)
•        Waterbeds, sofas, soft mattress, pillows, and other soft surface will be
prohibited as an infant sleep surface.
•        All pillows, quilts, comforters, sheep skin, stuffed toys, and other soft products
will be moved from the crib.
•        When a blanket is used, the infant will be placed at the foot of the crib with a
thin blanket tucked around the mattress, reaching the infants chest.
•        The infants head will remain uncovered during sleep.
•        Infants will be places in a supine (back) position for sleeping to lower the risk of
sudden infant death syndrome (SIDS). (Unless the child has a note signed from a
physician specifying otherwise)
•        When infants can easily turn over from the supine to the prone position, they
will still be placed on their backs but allowed to adopt whatever position that they
prefer to sleep.  Unless a doctor specifies the need for a positing devise that restricts
to movement within the child’s crib, such devices will not be used.  
•        If you have any questions about SIDS and infant sleep positions please all 1

Thank you for choosing my services for your child.  I started this business in 1996 in
order to provide quality, reliable daycare for my own child.  My goals are to provide a
positive, safe, nurturing environment to all of the children in my care.  I realize that
most parents in today's society do not have the opportunity to stay home with their
children, so I intend to offer the next best option.


LATE FEES                           Initial _____________
You are scheduled for childcare for the hours listed in your Contract and Rate
Agreement. If you drop off before or pick up after these times, you will be charged a
late/early fee. An overtime fee of $1.00 per minute will be charged if you are late
picking up your child or early dropping your child off. Late drop off does not
constitute late pick up. Payment of the late fee is expected at the time of pick-up in
cash.
It is your responsibility to have your child picked up on time. If you know you will be
unavoidably late, it is your responsibility to have an authorized alternate person picks
up your child. It is not my responsibility to tell you that I have plans after childcare
hours so that you can be here on time. Traffic or weather problems (except in
extreme situations) will not be an accepted late excuse and you will be charged
accordingly. Please remember that it is your responsibility to allow ample time to get
here to pick up your child on time.  
Continued late pick-ups may result in termination of childcare services.
Please keep in mind that this is an in-home childcare facility and time with my family is
very important. I would very much appreciate that you be prompt in picking up your
child each day.


Maternity/Extended Leave:                    Initial _____________

In cases of your potential absence due to maternity leave where you will be placing
your new baby into my care, all forms must be filled out prior to starting care.  Also,
half the regular weekly fees will be due for the new baby’s slot to be held.  Also due
will be regular fees for any other children enrolled under my care whether they attend
during this time or not.













Meals and snacks

Daycare will provide meals and snacks, with the exception of infant formula or breast
milk.  You are certainly welcome to provide any favorite foods your child may have,
but please provide enough to share with all the children. The Daycare is on the 4 c’s
food program which I am reimbursed once a month.

I will follow USDA guidelines when planning our menus.  I will offer the following meals
and snacks to all children in attendance at the times identified in the daily schedule.

School-aged children will be offered an afternoon snack upon return from school.

Monthly records of meals and snacks are available for your review.

Parents providing their own children's meals and snacks will be informed in writing of
the USDA nutritional requirements.

Children younger than 12 months must be served formula or breast milk, unless
written direction is on file from the child's health care professional.  I will provide
Similac advanced formula.

If your child has special dietary needs or has food allergies parents must notify the
center in writing.

Meal times:
Breakfast: 7:30 a.m. – 9:00 a.m.
Snack: 10:00a.m.
Lunch: 11:00 a.m.-12:00 a.m.
Snack: 3:00 p.m.
Snack: 6:00 p.m.
Sample breakfast: milk or 100% juice, whole grain toast, or muffin, and fresh fruit.  

Sample snack: milk or 100% juice, whole grain crackers and cheese or fruit,
occasionally, I will serve cookies such as fig bars or other home made fruit cookies,
however highly sweetened snacks such as cool-aid, soft drinks, cakes, candies will
rarely if ever be offered during daycare hours.

Sample Lunch: Balanced meal such as:
Milk, a main course such as a healthy sandwich on whole wheat bread (including
peanut butter and jam sandwiches, grilled cheese, bean burritos, etc)  or real
macaroni and cheese with vegetables, whole grain pastas with spaghetti-style
sauces, nachos with real cheese, beans  and vegetables, vegetable lasagna, and
home made soups etc..

A vegetable side dish such as veggie sticks and dip, or other steamed veggies, or a
salad

A fruit serving (fresh whatever possible) such as apples, grapes, bananas, kiwi,
oranges, raisins, strawberries, melon balls, ETC.
Menus are rotated every 2 weeks and are posted on the mail box bulletin board in
the entry way!

Breast feeding
If you chose to breast feed and bring pre pumped milk please bring it is breast milk
storage bags in a cooler bag (like a lunch box so to speak) please mark it with the
date for it is only good for 30 days in a refrigerator freezer and 6 months in a chest
freezer! I need to know it is safe for the child!

All food will be cut up in safe-size pieces in child-size portions.  Children may always
have seconds upon request, and no child will be forced to eat anything.  It is my
experience that children will eat when they are hungry, and will get all of the nutrients
as they need when consistently present with a verity of healthy choices.



My Philosophy:                           Initial _____________
As a working parent, you face the difficult challenge of balancing the demands of
work with the needs of your family.  Finding high quality childcare that is flexible to
your work schedule is one of the most difficult tasks you will face.
I strive to provide safe, comfortable, stimulating, fun, educational environment in my
home with a semi-structural day.  Activities will include free playtime, structural
playtime, large motor playtime, small motor skilled activities, small time and theater
time, arts and crafts, music and dance activities, nature and ecology awareness,
nutrition, cultural activities, and other educational yet fun activities.  I will also provide
nutritious meals, snacks, and encourage good personal hygiene and manners.

Television will be aloud during daycare hours, but only educational shows will be
aloud. Some examples are Barney, Blues Clues, Arthur, Franklin, Little Bear, etc...  I
also have a computer that will be available to all ages but only under adult
supervision.  School age children may have internet access, upon parental request,
but only again while supervised.
Through a mixture of guided and indirect creative play, children in my care will have
the opportunity learn about color, shapes, textures, numbers, letters, animals,
seasons, feelings, senses, nutrition, personal hygiene, manners, basic science and
math concept, and creative concept such as "pretend" play, art, music, drama and
dance.  These may all sound lofty goals for infants, toddlers, and preschoolers, but
most children learn all of these things and more through creative educational play.  I
do not believe in pushing any child to learn at an accelerated rate, and will not make
a child feel pressured to achieve developmental milestones.  By providing the right
kind of fun safe environment, I believe the children will learn at their own appropriate
pace.



Nap/Quiet Time:                   Initial ____________

All children are required by State guidelines to have a rest period each day.  I do not
require older children to sleep; they must however lie quietly until the other children
are asleep and then may quietly play or look at books.  School age children may play
outside or quietly in the playroom until rest time is over.  I do sometimes offer movies
during this time for children as well.

1-2-3 Family Day Care   will provide care for children between the ages of 2 weeks
and 12 years years.  I will never refuse to enroll a child on the basis of race, sex,
color, creed, political persuasion, national origin, handicap, ancestry or sexual
orientation.  All children will be enrolled for a trial period of 2 weeks.  During the trial
period either the provider or parent may terminate child care without advance notice.

A child may be discharged from the center for reasons such as, but not limited to:

        Failure to pay fees on time.  (Grounds for immediate termination, without
advance notice.)

        Lack of parental cooperation.

        Inability of child care program to meet the needs of the child.  I will consult with
the parent concerning how any problems might be solved before ending the care
arrangement.  The parent will be referred to other community resources.

        Repeated failure to pick up the child at scheduled time.

        Failure to complete and return required forms.
Parents must give a 2 weeks written notice of their intent to withdraw the child(ren),
and will be required to pay for those 2 weeks, whether or not children continue to
attend.  All outstanding fees must be paid.

I will give a 2 weeks written notice of my intent to discharge a child (and try to inform
parents of local resources that may be of help to them), except when due to parent's
failure to keep current with fees owed.  Should parent remove the child during a
notice period I initiate, fees will not be charged for remaining unused days.

Open Door Policy                           Initial _____________

At 123 Family Daycare you can always be assured that the door is open to you.
Please feel free to drop in and check on your child, however keep in mind a child just
adjusting to a new surrounding will want to leave with you when you pop in for a visit.
These early drop-ins should be made when it is appropriate for you to take the child
with you, or made when you can visit unnoticed as to not interrupt your child’s day. A
child going through separation anxiety will most definitely react by a parent coming
into the daycare and not taking them with them when they depart. Morning drop-off’s
and afternoon pick-ups are made easy by my allowing you self-entrance into my
home. Please give the children and myself the courtesy of a short knock and then
enter on your own. This prevents any shocked or potentially scared children by the
sudden appearance of an adult in the home. There will be times where I am pre-
occupied with a child or unable to hear the door. On the same note, If you arrive
before 5:30a.m. or after 5:30p.m. for any reason, please knock as a guest would
coming to my home. Do not enter freely unless it is during Child Care Hours, as a
courtesy to my family and I. You are also invited to call and check on your child
during the day. Please keep in mind there may be times when it is not convenient for
me to run to the phone. (i.e. diaper changing, bottle feeding etc.). If the phone goes
unanswered, please do not become alarmed, simply leave me a message on my
answering machine and I will then return your call as quickly as possible.

Open door policy does NOT mean that our doors will be kept unlocked. I believe that
it is extremely important to keep the doors locked for the safety of the children and
my home. I do not want unwanted or unexpected visitors to enter without our
permission or knowledge.


Personal Belongings                   Initial _____________

No toys should be brought from home.  Little ones have a difficult time sharing with
others, and it is even harder with their own special toys.  Exceptions will be their
“cuddly” for naptime.  Parents are required to supply diapers/training pants, wipes,
formula (if you choose not to use mine) or breast milk and a change of clothes.  In
addition if your child requires a special diet, you must supply it.

I will send home a notice informing you when your child's supplies at daycare are
running low.  I will also keep extra diapers and wipes here at my family childcare so
that no child would ever have to do without.  If you do not send in supplies to restock
your child's shelf within two days of your written notice, I will charge a fee of $1.00 for
each diaper that I use from my personal supply for your child.  Fees will be added to
the weekly tuition amount and need to be paid in full by the end of the week

Property brought to day care is not my responsibility.  I cannot and will not be
responsible for lost or broken items.  If the children are unable to share, their item(s)
will be put away for the day.  If a favorite cup, cuddly or a similar items is brought with
your child you will need to make sure you take it home if you wish for it to go home
otherwise cups are put in the kitchen and favorite items are in their cubbies.

Any property that remains 10 business days after the last day of the child's
attendance becomes the property of the 1-2-3 Family Day Care to be disposed of as
I see fit.  Please take your child's belongings on their last day.

Pets:                   Initial _____________
We love animals here, and so we have daycare pets! We have fish that the children
enjoy watching & feeding, and we also have 3 cats they have been raised in our
daycare and they feel that they are just like one of the kids. They are very gentle and
love the children. They are up to date on all their shots. You will be notified at least 2
weeks in advanced if we get another pet! Excluding fish!  The children will be closely
supervised when the animals are accessible to children to ensure that both the
children and the animals are protected from harm.
All my cats have been vaccinated.  If your child has pet allergies, please inform me


PHILOSOPHY OF LEARNING

My philosophy is that a warm and nurturing environment is made rich by “hands on”
play-learning opportunities.  Children learn about themselves and their environment
through their surroundings.

From birth, babies are marvelous learners, immediately investigating the sights,
sounds and feel of the world.  Long before walking and talking, they are exploring
their own bodily powers and what the world has to offer.  Though much of a very
young child’s play, learning is self-motivated and self-directed, it is still necessary for
caregivers to stimulate the child through individualized play-learning experiences in
an enriched environment.

Children of all ages are given opportunities to learn through experiences with stories,
songs, art, science, dramatic play, sensory materials and math activities.  A
preschool program designed for Family Childcare will also be provided for children
ages 2+.  Younger children will be considered at the parent’s request.

PROPER ATTIRE
Child’s play is messy work. Your child will be painting, playing on the grass ,sand,
sometimes water and various other activities. Please do not expect me to keep your
child’s clothes clean and free from stains. The latest fashions are cute and
appropriate for fancy dress and photographs but not for child care. The children will
want to participate in all activities regardless of their dress. Please remember to
replace your child’s clothing in their cubbies atleast every 6 months to assure we
have a garment that fits. Children typically do not wear shoes when inside the home.
This helps to keep carpets clean and the floors more sanitary for crawling infants and
children. It also helps when your child has easy on/off shoes that they can do with
minimum assistance.


REFERRAL REWARD PROGRAM                     Initial _____________
Referrals from a current client are one of the biggest compliments I can receive. As a
special thank you, 1-2-3 Family Day Care offers families a free day of care for each
referral that enrolls. Your free day will be deducted from your weekly fee after the
referred child has attended for 3 weeks. Rate of free day will be figured based on
your contracted hours of care.

This will allow you to have a part in choosing the children your child(ren) will spend
their days with.

Releasing Your Child                   Initial _____________
My normal procedure is to release the child only to his or her parents or someone
else the parents designate. If someone other than the parent is to pick the child up,
please notify me ahead of time. A verbal notice is fine on that day if this person is on
the list of those authorized to pick up your child. If the person is not on that list, I must
have written permission to release your child.
One of the forms you are required to complete designates who may pick the child up
if there is an emergency and you can not contact me. Please make sure those listed
are persons with whom you would allow your child to leave if that person showed up
at my door and said, "I need to take Johnny with me." Those on the list should also
be people I could call in the event something happened and you did not show up to
pick up your child.
Please inform your emergency contacts that if I do not know them and the child is too
young to recognize them ("Hi, Grandma!"), that I will ask for identification. I do not
mean to offend them. This is simply a measure taken for the child's protection.


RELIGIOUS PRACTICE                   Initial _____________

I do not include religious instruction or practices in my daily activities.  

We celebrate the following occasions: Christmas, Easter, and Halloween, 4th of July,
Valentines Day, and each child's birthday and many other holidays! Every day can
be a holiday! Like national cheese burger day secretary’s day etc...

Resolution of Parent concerns  initial _________

RESOLUTION OF PARENT CONCERNS
123 Family Daycare  is committed to work with diligence to address any
concerns/issues that may be brought by parents pertaining to the program or the
care of children at the site. We understand that parents have chosen 123 Family
Daycare  and Trust us with your most prized possession. (your child(ren)
123 Family Daycare  will do all it can to create a successful care arrangement for the
child and family. We respect the parent’s decision to find the care that they believe is
best suited to meet the needs of the family and their child. If a problem arises
regarding policies, the program, staff, physical environment.Parents should first
discuss the problem with 123 Family Daycare in one of the following ways: An
individual meeting during office hours or address the concern, in writing, directly with
Kris Steiner Owner of 123 Family Daycare. Parents are encouraged to provide
specific examples and/or information related to the issue to be addressed. It is the
responsibility of the Me (Kris Steiner) to address parental concerns, and it is
expected that I will make time and sincere efforts to resolve the issue to the parent’s
satisfaction. If you feel that your complaint was not dealt with promptly and/or
inappropriately, you may schedule a meeting with me to discuss the necessary action.
Resolution Steps
1. 123 Family Daycare will schedule individual and/or joint meeting times
with the parent with the goal of resolving the issue in question. Two meetings will be
held to resolve the parent concern.
2. If for any reason, the concern(s) is not resolved to the parent’s satisfaction, and
the issue remains irreconcilable, the parents may be asked to seek alternative care
arrangements. Parents will be given a two week notice in writing, but if it is in the best
interest of the program and/or the child, a shorter notice may beimposed.
4. The parents may appeal the dismissal to 123 Family Daycare in Writing.
If at any time that a parent(s) enrolled at the 123 Family Daycare believes that 123
Family Daycare is unable to satisfy an issue to the parent’s satisfaction, this policy
allows for a release from the child care contract with a two-week notice. The request
must be made in writing, signed, and delivered to123 Family daycare and will be
considered final. Tuition for this first week is required and nonrefundable. f
circumstances prevent a child’s needs from being met at the center, or if a child’s
ehavior is having a consistent negative effect on the well-being of other children, 123
Family Daycare may decide to terminate or limit that child’s enrollment.  Before the
child’s enrollment is withdrawn, I will work with the parents to develop a plan to help
the child be successful in the program.
Additionally, if a parent or guardian exhibits behavior which is detrimental to the
health and well-being of the children and/or staff in a classroom or negatively
interferes with the normal functioning of the classroom or program he or she may be
asked to terminate care with 123 Family Day Care
Families may not be allowed to re-enroll if they have a past due account balance,
Previous payment issues, unresolved classroom behavioral issues, or have not
followed 123 Family Daycares policies and procedures, as stated in this handbook.

Reserving Slots (new clients)                     Initial _____________
Due to a numerous number of requests for child care I do not hold spaces without
pay. Each space represents a percentage of my income. When you chose to place
your child in my care I will reserve the slot for you as soon as, but not before, all
completed paperwork is returned to me along with the $10 paperwork/ materials fee
as well as the full final 2 weeks fees (in this situation final 2 weeks fee will not be
spread out over first 8 weeks of care). This acts as a commitment to me on your part.
Until that time I will continue to interview for other clients. If paperwork and fees are
returned to me, I will hold your slot for up to 4 weeks for normal fee. This fee is due
each Monday morning. These fees are non-refundable, even if other child care
arrangements are found. Late payment policies will apply to fees not received as
described above.

Returned Check Policy                             Initial _____________
Parent will be charged $25.00 for a returned check.
Child care services shall be terminated if there are three late payments or three
returned checks in one year.

REVISIONS TO HANDBOOK AND CONTRACT

Every effort will be made to notify all parents at least two weeks before changes take
effect. I retain the right to enforce these policies at will. Lack of enforcement of a
certain policy at any given time does not indicate that that particular policy is no
longer in effect.

SAFETY

explore, learn and experience many different things. Some features that help insure
your child’s safety are:

*The hot water heater is regulated at 120 Degree’s.

children are present.  So please if you are a smoker please leave your cigarettes in
your vehicle.

If I suspect you are under the influence I will contact one of the people on your
emergency contact list (if I cannot get a hold of somebody on that list) I will contact
the Boscobel police department.  If no one responds to my call I will contact grant
county dept. of social services.  (This is so I am not held responsible for your actions)


TAX REPORTING                                      Initial _____________
I will supply you with a year-end summary of all daycare fees paid during the year for
tax purposes. This will be given to you in January of each year or when services are
terminated. You may request a statement at any time.
Throughout the year you will be provided with a receipt each time tuition is paid. It is
your responsibility to keep these receipts

Termination policy:                          Initial _____________
You are required to give 2 weeks written notice when you decide to terminate
childcare.   Payment for the two weeks must accompany the termination Notice.  The
2 weeks will be paid in full, regardless of whether your child is in attendance or not.  I
will give 2 weeks written notice of termination for which full payment is due, whether
your child is in attendance or not.
I reserve the right to give written notice of IMMEDIATE termination where there are
extreme circumstances that affect the well being of me or other children in
attendance.  Termination notices will not be accepted while Provider or parent is on
vacation.  The notice is to be handed to the provider I will sign it and give you a copy
at that time.  You must pay 2 weeks fees in lieu of 2 weeks notice even if the child is
not in attendance.  There is no 2 week grace (adjustment ) period.

•        Failure of parent to pay
•        Failure to complete required forms
•        Lack of parental cooperation
•        Inability of program to meet the needs of the child
•        You as the parent need to give me a 2 week notice in writing.  (If you don’t I will
be charging you 2 weeks pay.  If you have an ending balance I will take you to court
for the remaining balance and 2 weeks pay.  (if there is a ending balance then you
are required to have it paid in full, if not I don’t have to provide 2 weeks daycare but I
still get paid fo2 weeks.
•        I will give you a 2 week notice unless the problem is a danger to me and or the
other children in my care.  

Time Clock
SIGN-IN AND SIGN-OUT SHEET
Parents are required to check the child in and out daily using Time Clock! There is a
card with your child’s name and birth date in the card file located in the entry way of
daycare.  Parents are to personally escort their child in the center each morning and
inform me that the child has arrived. In the afternoon, parents must Clock out using
the same system and  notify me that they are leaving with their child. If the child is out
on the playground the above still applies.
There is a $3.00 charge per day per child for not signing your child in or out each
day.




TOILET TRAINING IS YOUR CHILD READY?                   Initial _____________
1. Expresses interest in coming into the bathroom with you to find out what goes on
there and perhaps even sits on the toilet herself.
2. Understands what the toilet is for and what it means to have a wet or dirty diaper. If
she also shows a preference for being clean and dry, fussing when she's wet, pulling
off a dirty diaper, or asking to be changed; all the better!
3. Knows the words for urinating and having a bowel movement (such as "going
potty" or whatever words your family chooses).
4. Can stay dry for at least two hours at a time.
5. Has regular bowel movements with soft, formed stools
6. Can and will follow simple directions, such as those for washing hands.
7. Can help pull pants up and down
8. Seems to recognize at least a few seconds ahead of time that she's about to go,
and can tell you before it happens. (Many youngsters will squat, leave the room, or
get "the look" before having a bowel movement.)
9. Is in a willing, receptive mood and isn't going through any major transitions (like
adjusting to a new sibling or school).
10. Demonstrates a desire for independence (for example, wants to be a "big girl"
and do things for herself) -- or, better yet, shows a specific desire to use the toilet
like mommy and daddy do!
If your child meets most of these criteria, she's ready to try. If not, wait a month or two
and reevaluate.
Potty-Training should begin at home over a long weekend or holiday.
Once you have had success at home for at least a week, your child may begin
wearing CLOTH training pants with rubber pants to cover them at child care.
You must still provide me with 1-2 pull ups for nap or other extended periods. Also
you MUST provide me with at least 2 complete changes of clothing for your child.
Under no circumstances will your child be allowed to potty-train in regular underwear.
This is for sanitary reasons! Regular underwear cannot contain urine & feces to
prevent the spread of germs in my home & to other children in care & to my family.
Please cooperate with me on this matter.
Also, if you begin training, please notify me so that I can continue with all the work
you have accomplished.
Further, if within 2-3 weeks, your child shows no signs of progress, I reserve the right
to put your child back in diapers & try again in a few weeks.
When you feel your child is ready for toilet teaching, I ask that you begin this
teaching at home. I will follow through and encourage your child while in my care.
Please keep in mind that the activity level here can distract your child from
responding to an urge to use the potty, more so than at your home. Therefore, I will
continue to use diapers or pull-ups until your child can and will announce that he/she
must use the bathroom (not just at home, but here, also) and can control his/her
bladder and bowels for a few minutes beyond that announcement. When the child
has reached this point, training pants (5-ply, not plain terry cloth) with plastic pants
may be used.
During toilet learning parents will need to supply:
•        three complete changes of clothing (socks included)
•        5-ply training pants (6 pair per day)
•        plastic pants (6 pair per day)
•        diapers or pull-ups for naptime
When accidents are only happening once every other week, 5-ply training pants will
be used without plastic pants.
Do not bring your child in panties or underwear until he/she has naptime and bedtime
control established.
I also ask that during toilet learning, the child be dressed in "user-friendly" clothing as
much as possible. The best items are shorts and pants with elastic waists, or dresses
for girls. Try to avoid really tight clothing, pants with snaps and zippers, and overalls
as often as you can. Your child will want to help pull pants, etc. up and down, plus
clothing with too many "gadgets" makes it harder to get the child on the potty in time


TOYS                         Initial _____________
Please DO NOT allow your child to bring toys from home, as it tends to cause conflict
with the other children. The exception would be item for quiet time such as a favorite
stuffed animal or blanket. This will only be allowed at naptime.

Trial Period         Initial _____________

All new children will be cared for on a 2 week trial period beginning on your child‘s
first day of care. During this time the Parent or Provider may terminate the child care
agreement with 24 hours notice. No pre-paid fee’s will be credited/refunded upon
cancellation during the Trial Period. After the Trial Period, a 2 week’s written notice is
required to terminate the agreement (see Termination Policy).

TRAININGS/AFFILIATIONS                        Initial _____________

I have been a family childcare provider since March of 1996.  I have completed my
Licensing Orientation to become a licensed daycare home for the State of Wisconsin
and participated in the state's Mentor Program.  The Collage Courses that I have
completed are Child Nutrition Mod 1-2, Infant and Toddler Basic Care, Child Care
Teaching Methods, Mentors and Protégés at work, Family daycare Licensing,
Marriage and family, Nutrition Health and Safety, Family Daycare Management 1-2, I
have also completed orientation for certification in Grant County WI.  I have attended
educational workshops pertaining to child development and have received a mini-
grant from CCR&R to put toward the improvement of my daycare.  I have also attend
WFCCA  (Wisconsin Family Child Care Association ) conferences to continue my
education.  Of course I am CPR and First Aid certified. Other Trainings include  
Mentor Protégé’ Teacher Program, 3 hours yearly nutrition classes, Child Care
Business Partnership program, The Nurturing Program,

Transportation

1.        I do not provide transportation to and from home, I will provide transportation
to and from school, and for day care field trips
      I do carry liability insurance on the vehicle used for transportation.

Occasionally, we may have the opportunity to take a field trip. All children will be
placed in safety-approved car seats as required by state law. Parents may be
requested to provide a car seat for the field trip. I document the Childs whereabouts
in the log book kept in the van. (Leaving daycare time and date   and arriving
daycare time and date) if we leave by stroller. (We go on walks daily!)  I have a cell
phone that can be called if you need to reach me and I don’t answer my home
phone!  See front cover of policy!
You will need to sign a general Travel Permission form.  At times, if a field trip
involves admission fees or lunch, you will be asked to pay for these fees prior to the
field trip.
If you do not wish for your child to participate in the field trip, you will be required to
make other arrangements for childcare on the day of the field trip. There will be no
reduction in tuition if you choose for your child not to attend the field trip.
At times, we may plan a field trip or it may be necessary for me to transport your child
by car. A permission form is provided upon enrollment. All traffic and safety laws will
be followed. No child will ever be left unattended in a vehicle. Also, because this
business is run out of my home, it may be necessary to run occasional errands with
the children. Parents will be notified at drop off time or with a phone call. The only
time we may leave without notice is in the case of an emergency, in which case a
note will be placed on the door leaving instructions where the children can be found.
Unless it is an emergency, you will always be notified prior to any outing from and
reserve the right to refuse. If I do not have your permission to transport your child,
you are expected to pick up your child immediately and make alternate child care
arrangements for that day.


Tuition and Hours of Care                     Initial _____________

Current fees appear on the rate sheet.

My services will be available Monday through Friday from 5:00 a.m. till 5:00p.m.
Licensed hours 5:00 p.m. County certified. Night p.m. care is by choice! County
Certified from 5:00 pm. To 9:00 p.m. prefer Day children. If I do night care for you it
will only be temporary!  Please discuss needs for care outside these times and I will
attempt to accommodate reasonable request at a possible additional fee.  (Although I
will do my very best to be available every business day, it is conceivable that I may
be forced to close occasionally due to vacation, illness, or other emergency.  If you
cannot ever tolerate, but possible lapses in care, you should consider placing your
child in a daycare center, or make other arraignments for backup coverage.  I can
recommend several options if you need referrals.

This is a Reminder I got from Social services regarding W-2!

Just a reminder to all W2 Childcare Assistance recipients:
The state will only pay for work hours and/or school hours when an individual is also
working. The authorizations are done by the week and any unused days that week
are not carried over to another week! Your daycare provider will not be paid for any
hours other than work/school hours for the week the authorization is for. If you
choose to leave your children with the daycare provider for other hours ie: shopping,
doctor appts. etc, you will be responsible to pay for these hours. Please do not
abuse this program or the funding may be cut and then the state will not be assisting
anyone with daycare costs. Thank you for your anticipated cooperation participating
in the W2 Childcare Program.

Unless other arraignments are made in advance, charges for the next week’s care
are due on Monday for that week of care.  
. (There is a $1.00 per minute early drop off, and late pick up charge. )  It is due on
the next schedules day of childcare.   Payments may be made by check or cash and
should be made out to Kristina Steiner. Checks must be dated for the day they are
due, no post-dated checks will be accepted.  

Tuition rates are according to the following hour categories:

State Funding:  I do accept state funding for childcare.  Please contact your worker
as soon as possible to give them my information.  I need to have a Notice of Decision
so I know what hours the state will accept for billing.  Any hours over and beyond
what the state pays for is your responsibility and you will be informed of any hours
you went over.   All co pays that the state says you need to pay are due on Mondays.

Fees are to be paid in advance on Monday for the following week's services.

If there will be a third party payment, as from an employer or the county, a special
payment schedule will be arranged and detailed in the contract.  Parents will be
responsible for any specified co-payments or unpaid amounts.
(EXAMPLE)   If Sammy is was scheduled for childcare from 8:00 a.m. till 4:00 p.m.
Monday through Thursday. That is 32 hours, Fridays is from 8:00 a.m. till 12:00 p.m.  
4 hours.  36 hours total! Sammy’s Parents or Social Services is paying for Sammy’s
daycare hours whether Sammy is there or not! Ok Sammy is ill on Monday so he
doesn’t come to daycare SOCIAL SERVICES will only pay for Sammy’s scheduled
hours! You can not just assume you can make up the hours some other time! The
additional hours are extra! If you are not working Social Services will not pay for
Daycare that day! You can not make up hours some other day! That is an additional
fee.
If you do not understand what is stated above please feel free to talk to me about it!
If you need to run to the store or run errands before picking your child up remember
there is a late fee. I take my children with me so you will have to take yours!  

There will be no reductions for additional children from one family.  I am limited to the
number of children that I may care for at one time.

Your child(ren) will be taking up a weekly slot! If you are scheduled for 40 hours 5
days a week then that is what you have to pay for.  If a holiday falls on a week day
(see paid holiday’s page 22) there will be no weekly price cut!

if your child is under 2 yr of age it is $60.00 per over night same time frame as stated

Use of Television Dvd’s and Videos
No child is required to watch tv. It is on most of the time but if I turn it off then they
want to watch it!

What Not to Bring from Home:
Candy, gum, and any other individual snacks. In addition, we will not allow toys from
home (except for infants). All of these things create competition among the children
because one has something that the others do not. If you would like to bring a snack
in for the entire group, please feel free as long as it is not candy or junk food and we
are aware you are bringing it. It needs to be a wholesome snack. If you have any
questions, please ask.

A FEW "CLOSING THOUGHTS": I am a professional licensed family childcare
provider and I am committed to providing high quality childcare in a safe home
environment.  Parents sometimes forget that my services are much like any other
business because I choose to work from my home.   I am not just a "baby-sitter" who
takes care of your child, so please treat me with respect. Please try to remember that
childcare is a "high stress" job, so I do occasionally need to take a vacation or just a
day off.    I've been told by some parents, "I wish I could sit home all day and get paid
for it!"  The fact is, I rarely EVER sit (much less relax!) I work over 12 hours a day,
and get little or no "breaks" and get paid less than minimum wage for caring for a
parents most prized possessions—YOUR CHILD!!
I do LOVE this job, though! I take pride in my profession.  Your children become part
of my own family!  My career in family childcare provides me with meaningful work
and extra income to help in supporting our family.   If you fail to pick up your child on
time, I have less time with MY family.  If you fail to pay your tuition, I find it difficult to
contribute to paying our bills.  If your family experiences an unforeseen difficulty, I am
very willing to work out a new child-care schedule with you.  However, I do ask that
you be respectful of my needs so that I can continue to offer your child quality care.
